I am about to start a new game plus for Mass effect 1 but is confused on how to change the difficulty to insanity. I see options for hardcore and insanity having to be unlocked but have no way to save settings with either difficulty. I am playing the 360 version if that helps.

Edit: This would be my third playthrough with my first being a vangaurd and this third one being the replay of my soldier.

Hardcore is unlocked after completing the game once. Insanity is unlocked after beating the game on Hardcore without changing the difficulty.

For New Game+ (NG+, starting a career with an existing character), you should be able to change the difficulty soon after the game starts.

Since this is your third time through the game, Hardcore should be unlocked. If you have not played through the game on Hardcore (without changing the difficulty) then Insanity will still be locked. If Hardcore is locked, it may be a bug, or you may have to play on Veteran without changing the difficulty to unlock Hardcore.

You have to make sure it is also set to Hardcore before you complete Eden Prime or else it will not count. Additionally, as Red stated, you cannot change it at all after that.
