Probably the biggest problem with any Mass Effect 4 is even though the current endings to Mass Effect 3 are quite similar they leave different results.
The most dramatic would be the technorganic future in the synthesis ending. Honesty, it probably lead to really odd changes in well everything. Plus the reapers are still around.
The control ending leaves the door open for the reapers to come back.
Then finally, the destroy ending shuts that door.
So, what I'm going to offer as a potential sequel is really a prequel with time travel. Basically if Bioware doesn't change anything, I suggest that whatever you did has an effect on Mass Effect 4 which is set in 2165 with you playing as Anderson, but something is off for long time players.
The tech level of Alliance Ships are suddenly more advanced than even the Asari and with more ships than the Turians. But even the Alliance is gone, it has been replaced by the Systems Empire, and humanity is raging a war of conquest on all other races.
Commander David Anderson who serves with the Imperial Navy wonders how humanity with its Krogan allies is able to beat back the Citadel races with just the Protherian ruins behind it. He is also upset about the slowly disentgrating rights for humans under the Empire as it seems that the Systems Empire readies itself for even a grander war. And then Anderson discovers a data packet from a group called Cerebus which details the alternate history of Mass Effect 1-3 and it alters to reflect your saved files.
Eventually Anderson forms a rag-tag group to find Lucifer who is behind the Systems Empire's government, and the big reveal will be this figure is a time traveling clone of the player's Shepard from the other two with memories implanted by Liara who was sent back in time to prevent the end of the relays.
"I thought as you did once, Anderson," said Lucifer. The figure got up from a chair facing many screens. "Only working together would galactic civilization to survive the onslaught that is coming, but then I had an epiphany. The Citadel Council never listened to me original, so why would they listen to me this time about . . . ."
Lucifer made quotations marks in the air. . . ."the Reapers."
Anderson was aghast. "You lived in that community of cultures. You saw their strength when they defended Earth against those Reapers."
Lucifer laughs. "Strength? They failed. The Council failed because they believed that they could handle anything given enough time, but the clock had already struck midnight as the Systems Alliance came into the galaxy. Due to their arrogance, everything that I cared about was taken from me."
Lucifer pulls out a locket that shows a picture of the love interest which is shuffled into a pocket."No if I was going to corrupt the past as a demon, I needed a seat of power to create a force that would defeat the Reapers."
Lucifer smiles. "You know Anderson. Stop this small rebellion against the Empire, and everything can be forgiven. I didn't offer that to Sovereign or the Collectors when I found them with my forces. The Reapers will come soon to discover why Sovereign has not opened the Citadel to them. I've already prepared a present for them at the Alpha Relay which will give them a nice surprise."
And the big choice would be rather Anderson joins with Lucifer, or if he decides through actions through Mass Effect 4 that he can stop the Reapers and Lucifer by upgrading his ragtag band alliance with advanced tech and by gaining his allies' loyalty.
