Naming characters has become far less interesting with the advent of full VO.  You no longer get the thrill of reading NPCs addressing you in the name you've chosen.  (I liked to go 'meta' and name myself "Protagonist" or "CharName.")

Yeah, VO really ruined a lot of things. I guess some day they'll try synthesizing the name, but then people will complain because it puts the accent on the wrong syllable or something.



I miss the days of reading things like "Come here, Blowhole, we've found a cave!" so now my chars have more normal names.

I named my first character, a Human Noble Warrior, Quisten. It's a portmanteau (or "smoosh" if you're a fan of "Lucy, Daughter of The Devil" ), combining the names of my sons, Triston and Quinten.
My second character is an Elf Mage named Euladan as kind of a shout out to the Tolkien-esque elf names. It just sounded right.
If I have a choice of first and last names, I usually name dwarf characters Thyroid Glandular. It's funny, and it has the bonus of even sounding very dwarven!

It depends...

If there is a default I often choose that though in games like DA even though there IS a default I mostly ignore it.

I've used Nimzar, Graham, Emil, Anlynnis (or An'lynis) for males more than once.
I've used Celeste, Tallitha Aers, Kathrynn, and Ae'lothk for females more than once.

However, I'll often invent/choose a different name based on what I think of the character.

Once I started a game (Dragon Quest V btw) and named the main character Graham. About 10 hours in I realized that the name didn't fit I finished that play-through anyway but delete my save file to start another run with a better name immediately afterward.
