I could talk to her the 2 times she was there in the Refugee Camp with Paragon options in check, but after Cerberus was there she's just gone forever - WHAT HAPPENED?!

Yes, the lack of her name on the wall was a bug on my part. I didn't know about it until someone posted the issue on the forums here. I'll be trying to get the fix into a patch if possible.
