Map: Firebase Dagger
Enemies: Cerberus
I was playing a human female vanguard and tried to charge an enemy when I was close to death. The charge animation began and the sound played but I was shot and killed while it was all happening. A nearby teammate revived me and thats when I started to teleport into the air. Usually I just go into cover to try to fix a bug like that but it didn't help I was pulled out of cover and started to teleport on top of it and back to my original position repeatadly.
As far as I can tell, enemies actually had to aim upwards to shoot me and I had to aim down to damage them, so my character model was probably flying at that point. While I was trying to be helpful to my team for the rest of the match despite the shakey screen, I ended up outside the actual map. Thats when I started to repeatadly fall through the world. The first few times I was teleported back to the starting location but the problem persisted. Then I tried moving away from where I was falling through the world and I fell sideways through the world in a manner that had my character falling for the rest of the match. I got pretty far from Firebase Dagger, I couldn't look up and see it by the end.
Also, if you happen to be one of the two guys that were in that match, sorry you had to go the last 2 waves alone.

Im not sure if it happen in same map, but I just got this problem, so I had to quit the game. However I wasnt die when charge, instead I was in process to revive team before killed, then I got this shaky screen after been revived by team. Lastly I was pushed out of cover when I try to solve the problem and get killed/revived again this time I was automatically ascent through ceiling and screen become messy

I just got this problem too on Firebase Dagger. I was playing with the Asari Vanguard, I charged at the same time a Banshee threw its Biotic Attack causing me to die. When my team mate revived me I spent the rest of that wave and the next couple of waves shifting up and down in the air (could still walk, run, attach etc) kinda like what you'd expect if there's a problem with your internet connection, like lag. However when I died during this phase, a team mate revived me and I spent the rest of the match just falling through the ground, I could use my Stasis attack but it was useless as I wasn't able to move or target anything, just falling into infinity!
At least I got the XP and credits when the match ended!

I have this same problem. I have noticed though that I still can be killed via melee while I'm floating out there. This suggests that as far as the game is concerned, that I am still at the original location of my death and what I am experiencing is a client-based "out of body experience". This also explains how my teammates can walk up to my now distant corpse and revive my soulless body. Since this has happened to me quite a number of times, I would certainly appreciate a bug-fix.

It happened to me twice, and all I did was rolling, trying to evade attacks. One other time I flew up into the sky, it was kinda fun to shoot at enemies and they don't realize you're up in the air... At least until I ran out of ammo.
