They also show up at almostly every mission you go to -- either right as you do, or right before/after you do.
So was this an unrealized plotline? Or, did I miss some part of the game?

This one, I'm not going to call out as a plot hole.
I think the entire point is to get you to try and figure if it was the Dalatross or Udina.
Now Udina is revealed as the traitor, however that kind of bait and switch is what the Salarians eat for Breakfast. Frankly it is just as believeable that the Salarians tipped off cerberus and Udina launched his plot as two completely seperate unrelated things.
