Ok so first off I loved the ending think it worked well and here is my opinion in the ending.

So we see the laser hit the team and suddenly Shepard is beamed to the crucible/citadel, but he isn't moved physically it is his consciousness that is moved, I believe Shepard is the catalyst as the child refers to him as 'the first organic'

Obviously Anderson is shepards conscous mind and Tim is the indoctrination trying to take him over.
also listen to Anderson's dialogue shepard is reassuring himself that he is doing the right thing, also the blackness that starts to creep in is the mmanifestation of the indoctrination
The child I believe is either a avatar for the citadel/crucible itself or its keepers ( as when you first come round there is a keeper watching you) and even though indoctrinated themselves what if they have enough free to offer the third choice synthesis which all though is what the reapers have been trying to do they see it as the universal answer that will stop the cycle but at a high cost.

So you choose control synthesis or distruction and this is where I think the vision element comes in.

In quite rapid sucession you see shepards choice, the Normandy outrun and crash/ be destroyed by a mass relay and then a story teller talking of the shepard.

What if the crucible is a prothean  beacon like on Eden prime or virmire, but instead of showing what happened before what will happen next? Forwarned is forarmed as they say knowledge is power and shepard is a wepon so to spea. So everything you see after making the choice is a vision and has yet to happen - then shepard gasp and comes to ready to say there has to be another way we can't destroy the relays and then in either the next dlc or mass effect 4 find away to deal with the reapers.
