Large information is released by a large pulse that drains the physically-allocated information in it.
After, it remains in a basic sphere form that can sit on a desktop, logically if one knew how to refill the sphere, they could use it as storage again and it would become large depending on the amount of information stored in it. The reasoning behind this type of storage must be due to the massive amount of storage possible rather than just on a data card of some sort.

In ME1 it is clear that when you touch it Shepard is filled with images and thoughts.
So best guess would be is that it is a large 'memory shard'.

Also, where did it go? It was in Shep's cabin as far as I remember. Did it get mistaken for a goofy paperweight and end up in a waste-paper basket when the Normandy got refitted?

I recall an e-mail from ME2, but didn't get one in ME3.
Why would you? The device no longer stores information, the pulse was harnessed by Cerberus intel. What else are we supposed to do with it?
It probably got transferred to R&D.
