kai leng, hands down. he is not even an enemy, he is a cheap, lousy capcom boss with invulnerability moves, phases and support-troops spawning during recharge. And he even repeats the same lousy "cover me while I reload" trick in both encounters.
2nd worst:
dr. eva: quick time event. totally out of place. marauder shields is much better.
mobs:
I would say guardian because they are just so annoying with their constant smokebomb spam, not because they are hard. I can respect engis fighting dirty and banshees having one-hit kill moves.
But Guardians....I have shields and the best equipment....hides behind smokescreen...

Reapers. I hate painting the target on the Reaper on Rannoch, it's just irritating having to roll around to dodge the damn particle beam all the dang time. The one on Tuchanka is annoying too, having to run for the thresher maw summoning bells while dodging the Reaper's leg stabby maneuvers and the Brutes running around willy-nilly, deftly avoiding those same maneuvers.

The Catalyst, for sure:
- it can insta-kill an entire game trilogy in 14 lines of dialogue! Somebody nerf it, please.
- you get a bottomless clip in the final boss battle, and yet it's still invincible. Game is clearly broken.

kai leng. nothing made me more angry at the game than his very existence, and continued besting of me in cutscenes, despite my regularly shooting him in the face.

The only time I died on Insanity that was on those last banshee's in London.

Marauder Shields -- Nevar Forget...
Seriously, on insanity you just fought through an entire game only to go through a cutscene where you get up...then you gun down three husks and meet a marauder who proceeds to gun you down if your really slow on the head-shots.
Inevidably, he kills you and you go back to the cutscene, get up shoot the husks and Shields guns you down again.
I think this guy killed many an insanity first play through since people would reset the difficulty to see how it ends (Not mine though).
Then comes the realization he was the final boss, ending rage, head through TV/Monitor, how Shields wasn't really an awful fellow for trying to stop you, and other things discussed in other threads.

Phantoms. It's a cool enemy and all, they just jump around too much. Really annoying how they dodge powers too.

as an eng8ineer whenever I saw an enemy turret I was happy cause I could just hack it and have it do the work for me
