One: In the refinery map, Bronze Challenge agains Reaper forces, at some point my female Vanguard ended up on top of a Brute. And then could not get down. Died, was rezzed only to end up off the ground again, clipping and unable to move properly, shoot properly or pick up ammo, activate switches, etc. Lasted the last two waves that way.

Two: on the Tuchanka cannon map, at some point my male Vanguard character turned essentially invincible, but I was unable to use powers or pick up ammo. I could not damage opponents through melee and just sat at an Atlas' legs using heavy melee to keep it in place as it could not move around me. Eventually, it was somehow able to hit me through some kind of aoe attack (maybe) and kill me. Once rezzed, things went back to normal.

3 waves of not being able to reload, use powers or equipment, melee, take much damage or do anything useful really was nice and frustrating.
