I was in the second dream, the one following Tuchanka, when I experienced all of this.
I understand that taking what I said at face value can be tough, but that's no reason to discount it. If you truly believe that I am wrong, then go play for yourself, and not for the 30 seconds it takes to catch the child, really LOOK, explore for awhile, you'll hear it I promise you. Just take a few minutes to test it before you condemn it

His wife was named Irikah.

I heard, but not directly. It frightens a little.
The "don't trust him" part is fuzzy, but there is a LOT of "Shepard" whispering going on in there. It is really, really creepy.
In the third dream, you can hear the Illusive Man's voice. Speculate on that if you want, it wigs me out.

No, I romanced Kaidan. It's kind of wierd I got that quote because I thought Thane only called you Siha if you romanced him, but maybe not.

yea i heard that on my second playtrue and i almost got a brownpants moment gona make a playtrue now and frap all the dreams becouse i cant hear all what they are saying maybe it will be easyer if fraps and you can play it over and over...

But just a thought..
Is it possible that BioWare have put in some hidden clues throughout the whole ME3 to help us figure out what the current endings are all about?
I've seen posts that mention other things that are 'strange' in other parts of the game.
If that's the case, and these have some meaning as it relates to the ending, then I'm very impressed.
After all, they did want everyone to 'speculate' for a while and perhaps also see how many will solve or get close to solving the 'puzzle'.
