So I completed both wardens keep and shale add-on. i now login and both the staff and armor (from sophie) is gone. Did the new patch go awry or did they just stealth nerf the game and not let anyone know?

I had that happen.
If your game thinks it needs to log in to authorise the DLC (digital versions do - physical versions aren't supposed to but some do due to a faulty install), and you resume a saved game before the client has confirmed auhorisation of Warden's Keep, then the Warden's Keep loot will disappear.
Do not save the game. Exit the game, and then verify from the main menu that Warden's Keep is active and installed. Then reload a saved game wherein you have that equipment.
If you saved your game without the equipment and then try to resume after re-adding Warden's Keep, your gear is still missing but Warden's Keep reappears as a new quest (so you could do it again and get all that XP a second time).
