1) The citadel was not over earth for that long, and the Reapers are not necessarily rapid in their harvesting.
2) When the citadel "blew up" it's likely that it didn't fall to Earth but rather just continued to orbit in multiple pieces. It was in geosnychronous orbit after all, plus Shepard wakes up alive which wouldn't be possible the citadel fell to earth.

You see multiple catastrophic explosions and power loss, the Citadel arms begin to move and buckle.
Given the huge explosion at the core the mass effect fields are most likely offline, which means it's going DOWN.
The Shepard breathing scene makes absolutely no sense in any context so can't be used as evidence.

Its plausible to assume even when the citadel blew up maybe the arms each have they own system that lets them keep life support and heck the keepers who knows maybe over time they fix the citadel with the aid of the fleets
But there is one fact many people keep forgetting, the citadel isnt destroyed in the control ending. It closes up but it doenst blow up. Heck when it comes to the control ending this comes to mind.

Also i know its been said that only a few people got out, but I believe that depending on how high your citadel war asset was then you had a better chance of getting more people out

Not when the Citadel explodes (unless you choose the Control option). Woo, Shepard commits genocide on the Keepers as well!
I hope you had fun helping Conrad Verner, the Council, and everyone else you've ever met on the Citadel. Because they're all dead now, regardless of what you did.
...Except for Aria of course, who needed to escape earlier on so we could get a Retake Omega DLC!

In the destroy ending, Shepard was on the citadel.
Shepard wakes up, decidedly not smashed to atoms after crashing into earth or dead from asphyxiation.

Personally, I think there are four noticeable "types" of war asset:
GROUND - which should determine the strength of the ground "hammer"forces, obviously.
FLEET - Determines the strength of the "Sword" and "Shield" fleets. (sub-option, SHIELD is divided from rest and determined specifically by the Geth/Quarian fleet strength)
CRUCIBLE - Determines the effectiveness of the crucible itself, based on scientists, engineers and unique research picked up by Shepard on the journey.
CITADEL - Determines strength of resistance on the Citadel when the Reapers take it.
As my calculations (current save file) go, the roughly estimated maximum for each division (Though they can be higher I'm sure, especially GROUND, with the promoted MP characters) is as follows:
GROUND - 2000
FLEET - 3000 - 3500
CRUCIBLE - 800-1000
CITADEL - 200 (if you include Spectres and a couple of other things)
You could easily create variable endings and end-games based on the individual strength of each force as the ending progressed. When Shep gets to the citadel to turn on the crucible, a high citadel score could result in running into C-Sec resistance trying to get survivors off, including the Council and various other tertiary characters . . .
EXCEPT THE ENDINGS NEGATE THIS COMPLEXITY SO WHATEVERS!

So everyone on the Citadel is dead and every form of life on Earth is gonna die when the Citadel crashes down and kicks so much debris into the atmosphere sunlight is blocked for decades. Shep has turned genocide into an art form.
