On my Infiltrator, I'll revive them almost all the time as I can just sneak past the mobs then get the hell away.
But on my other classes... only if I can get away with it or I can ress 2 players before getting downed myself.
When ressing people, there are two main factors that come into play:
1. If I can ress the person without getting killed myself, then I'll do it.
2. If I get knocked unconscious, it is still worth it if I can revive 2 or even 3 of my teammates.

Last night though had the case of letting a guy die intentionally as he was bieng an **** over the mic saying how awesome he was and he didn't need the rest of us and he was getting all the kills (Salarian engineer with a widow of course).
He died. Several times. I never revived him. I was the lone person (Asari Adept) to complete a wave with the rest dead, and we even only got a partial extraction at the end because I lured the nasties away and doubled back to the extraction zone, me and one of the others are the only two that made it.
So yeah, if you aren't an **** I will try everything I can to revive you. Been in a number of situations where we just traded revives because we coudn't survive the group otherwise.

I don't think there is some guide with every action to take according on the situation. It really depends on what's going on and if this is worth the risk.
By default I will always go this route:

and do my best either by rezzing or offering suppression fire to take some heat off your back. But when the cost is too high, if you died in front of a turret with a couple of phantoms wandering around you will have to wait for the place to be clean, I think that people now understood that you are not protected by a magical shield because you are rezzing someone.
There is also a time when priorities come first, if the timer is telling that there is little time to activate the last couple of objectives, I won't think twice about going:

Which can be pretty hard when you are actually leaving to die someone who just rezzed you a few times earlier.

I tend to run Infiltrator a lot on Gold, I love using my Mantis 10 with Damage Mod 5 + Cloaked Headshot and I've let people die and took the long road of guerrilla warfare to finish off a round by myself. But I've also gone balls to the wall and thrown grenades, shot missiles and used sabotage combined with cloaking to try and revive groups of fallen comrades.
Leaving a comrade to die in dire situations is never an easy thing to do in a coop mission. But both my friends and I have come to a much greater understanding that sometimes its best to leave them behind and complete the mission objective. It doesn't help if everyone dies and you lose the match. But if you can accomplish your mission objective and then wipe the enemy out, you revive your fallen comrade. Thus saving the day and not adding to the loss. Its a tough call, but If you're seriously surrounded and have no rockets in inventory, get the hell out and run. If there's an objective to complete, pay attention to the time limit and what needs to be done. Take out targets one at a time and don't get surrounded. Trust me, your teammates will congratulate you.

Can I kill the enemies in the proximity, so I can safely revive (and get to the nearest cover)? If no, sorry, I'm out of here.
I had a game earlier where I (foolishly) rushed towards the objective, and didn't notice the bunch of Cannibals/Marauders hiding behind large crates, so I got cut down. Few seconds later, another teammate comes to revive me. And dies pretty much instantly. And then in runs third teammate, and gets shot in the same manner.
Wish there was a suicide button so you could avoid people from even trying

Best answer for that is comms. Use it if you've got it. If you don't then. buy one if you can afford it. It's great to have comms. I use it sometimes with randoms and sometimes they can hear me. It helps.

My worst death, on Silver, I accidently got myself caught by a Brute trying to be sneaky running to get an objective. I wasn't at the objective, and when I died. as I lay bleeding out all I could see was the Brute stomping back and forth over and over on me.
Thankfully, we all had mics so when I heard a teammate say 'I'm coming, hold on' I was able to tell them not to bother, I was meat. They finished the level.
So the moral of the story is. Get a mic.

I make a split second flight or fight decision. Are you being lorded over by big nasties? Are you in the middle of nowhere away from the team?
If I can, i'll save you because that's what i would want somebody to do for me, but i'm not going to risk the team being down two instead of 1.

I fail often though with holding off.
Being cornered in a tiny dead end with a Brute blocking and killing multiple times even when cloaked, just because you revived someone once again is bad.

I had to let the my team, and the guy who picked me back up, die on wave 10 of a silver game in order to get away. But I finished the round alone and my team went on to full extraction.

If you can save your squadmate without getting hit or at the very worst case lose your shields do it.
If he's inside or nearby an objective, do it.
Otherwise no.
