Fatalaties occur based on EMS level and not on ending choice.

Because the citadel and relays dont cause a blast when they explode they implode after releasing the crucible energy blast which is not an explosion.

And what most people seem to ignore/not take into account, is that IT states, that Harbinger was attempting to fully indoctrinate Shepard, since it was a very fitting moment to finish Shepards indoctrination.
