I notice that I'm still missing the commendation pack even though I'm sure I have completed extraction at least twice during that period.

Did you by any chance have the option checked that ME3 can't send gameplay feedback to them?

Nope, It has always been checked.

best option was to use live chat with tech support to get the packs activated...
im assuming this was regarding operation goliath...
anyway... dunno man.. its been like 2 weeks, but wish you good luck on that

Gone through that, was told to re-install ME3 and see if it works.
