Try taking out the EC and see what it does.

Another fun glitch is when I use an Ops pack and I die immediately after. Then, somehow, I come back to life and I am missing a medi-gel.

Haven't encountered those issues and I use arc grenade a lot in mp. Does it happen frequently throughout a match or does changing classes do anything?SgtAmazing wrote...
Most recently on the Xbox (and PS3) in multiplayer I have noticed an error with most grenade throwers, more specifically, arc grenade throwers. I will push the correct button to use an arc grenade. Now at about 20% of the time nothing happens at all (yes, I have checked my grenade count,) or my character goes through the motions, but I do not throw a grenade, the number stays the same.
Another fun glitch is when I use an Ops pack and I die immediately after. Then, somehow, I come back to life and I am missing a medi-gel.

Another bug I experienced today in MP for the first time was a Bronze match switching itself to Silver mid-mission - a legitimate Bronze match (listed as Bronze while in lobby) started normally, and by wave 6 or so, everyone began to glitch: enemies floated above ground (and were also invincible), other players looked frozen, I could move and use powers/shoot but was really jumpy and kept being hit by invisible enemies - died repetitively but mysteriously revived again only to be hit by invisible enemy... hosting player left at this point (I'm assuming they were disconnected), and the wave restarted obviously in Silver mode since there were 2 banshees, 2 brutes, 4 ravagers and multiple marauders right from the start of the wave. Just as everyone was dead and we thought the mission would fail/abort, a new player spawned in the middle of a pack of reaper bosses, so we got to spectate the poor person get ambushed and almost immediately die.

Seems like the only way to get what I asked for is to host the match and wait until people show up.

2) Trouble with vorcha flamer (i only have soldier, idk about sen). It seems to not register when I am pushing the LB button. Y and RB work perfect though. A friend has same issue.
3) Being revived and immediately start running in opposite direction of where you want to go. Also happens at worst times on harder difficulties. (Almost looks like animation glitch Shepard has when talking to Liara in campaign where Shep's head follows glyph and is stuck at odd angle until conversation is over, only running like that...)

was rearranging the tv and the xbox, turned off the xbox while i was in the multiplayer menu...
then got back in to multiplayer and lost a couple characters like the N7 shadow, fury then I happened to have new characters like the QMI, Phoenix vanguard/adept,...turned off my xbox again then had my characters again.

In the EC normandy extraction scene at the beam, while saying goodbye (LI), as he's boarding the ship, Kaidan's helmet is off, then on, then back off through the scene.
About the Normandy EC scene do you have it set to where they wear helmets in convos?
I believe I do have it set to remove the helmets. Would it help if I re-ran the ending with the option on and then off and report back if there is any change?

Sorry it took so long to get back to you on this, I got caught up in a DA:O pt. I uploaded a vidoe of this captured from my xbox, the problem did happen again in the same manner:
(this Shepard was a pre-patch face import. bleh)
It seemed to be one line - when Kaidan says Don't leave me behind. Not game breaking but weird. it only happens with the squad helmets set 'off in conversations'. I put in the video all 3 options:
1) off in conversations
2) default off
3) default on
4) glamour shot of Shepard from ME1
Also noticed 2 other small glitches, the weapon changing when going to cut-scene (in part 3) however, that seems to happen in all ME games, so no big deal. Also in the rover on the way to the beam, when Shepard replies to Anderson the screen was empty but she was speaking (I have vid of that if you need it). Again, nothing game breaking, but worth note.
