Refusal simply does not work. It's obvious you've made Refusal + high EMS = "true ending" (infinity plus one sword etc.) and that is precisely what they were trying to avoid. Every other ending simply means "sorry, you don't have enough EMS (and made the "wrong choices"), go back and make better choices" just like anyone dying in the suicide mission = "sorry, you did the loyalty math wrong/took too long, go back and make better choices."
Otherwise from that, the rest of this is brilliant.
I think we can get away with a Refusal ending of this sort, because, as you stated, "just like anyone dying in the suicide mission" = "go back and make better choices". That means Bioware GAVE us the option to let anyone die in the Suicide Mission, which signifying they'd be willing to and have already presented a choice in the Mass Effect trilogy in which there is clearly a best outcome. Secondly, it promotes further replays to achieve this optimal outcome.
Meaning that no ending matters until you've done that "perfect replay." There's no artistic value there at all, no point in weighing Control vs. Destroy - either one just means you've screwed up and either suck at the game or suck at choosing X fleet for Y task.
