I know there are steps to save Miranda from dying on the Horizon, but for what ever reason she dies at the end of the mission, even after I have given the info on Kei Leng and the Alliance resources. What am I doing wrong or leaving out?

Edit: I also gained her loyalty in ME2.

did you romance her... (this shouldn't be a factor).

I would open up the save game editor and modify the romance if you haven't, see if it affects the end result in any way. It is quick and easy, and may get you the answer you seek.

Yes I did and broke up with her. I am on xbox, but I will find an xbox save editor to help.

Hmm... the romance thing shouldn't be a problem. Did you meet with her all three times before doing the mission? Twice in person and once by comms in the spectre office?

Miranda didn't die on my FemShep playthrough, but she did with my male Shep (he romanced her in ME2, but then broke up with her in 3 to go back to Ash). Not sure if that's a factor, but other than that, I have no idea what I did the first time around to get her to live. :/

Damn... Is this 100% confirmed? I was just about to download a 360 save editor (deleted the old one on my PC) to mess with the romance option and comfirm that I did everything.

I can confirm as well.

During my first playthrough (a soldier who romanced Liara in ME and Miranda in ME2) I broke up with her to go back to Liara. And even if I met her every single time she asked me to, told her about Kai Leng and gave her the Alliance intel, She died on Horizon.

However, in my current playthrough (a vanguard who romanced Ashley, then Jack), in which I did everything mentioned above (except the break up, obviously), Miranda survived.

Since the romance is the only thing that differs from my first save to the other, I figured it had to be this.

if you break up with her (if romanced), she will die. i can confirm that. and i believe the guide says so too. but if you reaffirm your love for her in the first convo, then switch li. talk to your new li before see miranda bout access. she will go to friend and survive. i think it a bug...

Here's is both rounds of my playthough. First you gotta hear her story first time you met her, second let her in on the Spectre intel this way she's basically safe and finally if you did those two your basically kept her alive. I didn't romance her in my insanity run but my first run I did so I can confirm that. Just listen to her and let you in on that intel and your golden and no Miri on the wall of the fallen.

Yes, if you lead her on then lock into another romance then she survives Horizon. That is if you gave her the intel.

From the wikia:

Finally, go down to the far end of the hall where you can salvage 4500 credits of research data. Then go right where you'll find an elevator. On the other side, you'll find a cutscene, with Henry Lawson holding Oriana hostage; if Miranda is present, she'll be at the standoff as well; if she wasn't warned about Kai Leng, or if you didn't complete Miranda: The Prodigal, she'll be gravely injured. As always, there are multiple potential outcomes.

If you ask about his legacy, the camera will focus on the cracked glass behind him.
If you can't pass the Reputation check and "just want Oriana," you'll have a Renegade Interrupt option to wound her in the leg. If you don't take the Interrupt, Oriana will backpedal towards the glass and throw herself and her father down, which ends in both of them dying. If you take the Interrupt:
If Miranda is present, she will slay him via biotics and Oriana lives.
If Miranda is not present, Shepard can kill Henry with another Renegade Interrupt. Otherwise, Shepard will tell Henry to run away, letting him go.
If you do pass the Reputation check, you'll be able to use a Charm/Threaten option that gets Henry to release Oriana. If so, you'll get a Renegade option that allows you to off him. If Miranda's present, she'll take it before you get a chance to.

If Miranda is wounded here (you didn't warn her about Kai Leng) (you did warn her but she was disloyal to begin with), she will then die of her injuries. (Note: if Miranda is present, she may appear wounded in the cut scene, but can still survive.)

this is incorrect i did warn her of kai and she was loyal to me in me2 after her quest   she dies no mater what to either her wounds or is shot after she pushes  her father out the window there basically no way to save mirande since  their si only 2 actual deaths in the game  thane and miranda

i dont think so if there is i played  thru multiple times and  there is no way to save her she  one those must die characters like thane, i will go play me1 thru m2 and see if i can romance her  and then try  me3 but i seriously doubt it will not let me save her

Weird. Maybe the guy was being head canon-y but I am so sure I saw a Mirandamancer gloating a while back about how his romance worked out great cause Miranda was stranded on earth with him as to the other romances that often get stranded on an island.Posted Image

Edit: Actually head canon seems like the likely culprit since we really don't know where Shepard is, Citidel or London.

yeah it does seem to be head cannon for her case   that they kill the creator and   his monster{i.e monster being Miranda} i would love to be able  to get her to live but.. i think they wrote  in such a way that there can only be one outcome

Maleshep, romanced her in 2, cheated on her in 3 but she still acted like she was my only love and didn't die. I didn't know she could die.
