Damn... Is this 100% confirmed? I was just about to download a 360 save editor (deleted the old one on my PC) to mess with the romance option and comfirm that I did everything.

Finally, go down to the far end of the hall where you can salvage 4500 credits of research data. Then go right where you'll find an elevator. On the other side, you'll find a cutscene, with Henry Lawson holding Oriana hostage; if Miranda is present, she'll be at the standoff as well; if she wasn't warned about Kai Leng, or if you didn't complete Miranda: The Prodigal, she'll be gravely injured. As always, there are multiple potential outcomes.
If you ask about his legacy, the camera will focus on the cracked glass behind him.
If you can't pass the Reputation check and "just want Oriana," you'll have a Renegade Interrupt option to wound her in the leg. If you don't take the Interrupt, Oriana will backpedal towards the glass and throw herself and her father down, which ends in both of them dying. If you take the Interrupt:
If Miranda is present, she will slay him via biotics and Oriana lives.
If Miranda is not present, Shepard can kill Henry with another Renegade Interrupt. Otherwise, Shepard will tell Henry to run away, letting him go.
If you do pass the Reputation check, you'll be able to use a Charm/Threaten option that gets Henry to release Oriana. If so, you'll get a Renegade option that allows you to off him. If Miranda's present, she'll take it before you get a chance to.
If Miranda is wounded here (you didn't warn her about Kai Leng) (you did warn her but she was disloyal to begin with), she will then die of her injuries. (Note: if Miranda is present, she may appear wounded in the cut scene, but can still survive.)

I know there are steps to save Miranda from dying on the Horizon, but for what ever reason she dies at the end of the mission, even after I have given the info on Kei Leng and the Alliance resources. What am I doing wrong or leaving out?

i dont think so if there is i played thru multiple times and there is no way to save her she one those must die characters like thane, i will go play me1 thru m2 and see if i can romance her and then try me3 but i seriouly dount it will not let me save her

Weird. Maybe the guy was being head canon-y but I am so sure I saw a Mirandamancer gloating a while back about how his romance worked out great cause Miranda was stranded on earth with him as to the other romances that often get stranded on an island.![]()
Edit: Actually head canon seems like the likely culprity since we really don't know where shepard is, citidel or London.
