I did a quick search for this, but couldn't find anything related.

I started up another game recently, where Thane lived during the suicide mission in ME2, and he appeared as he should in the hospital on the citadel, I talked to him a few times and such. However, during the cerberus attack on the Citadel, the game seems to act as if he is dead and he doesn't appear at all.

This hasn't happened in any of my other playthroughs so I am curious if its just a bug related to this save or something, or even if anyone else has experienced this.
