I think I understand the logic behind the ending, some of it may stretch a bit, but here goes.
A million years ago... An advance civilization creates the the Relays and the Citadel as its central control. They build an AI and a fleet of AI ships to take care of said Relays. However, they didn't like their creators for some reason and rebelled. They couldn't just wipe them out because of some logic law(i.e. Asimovs 3 laws), so they found a logic around that, convert them to a new form to make way for the next ascenders(i.e. new advance civiliztions). The Builders came up with a plan, to attach a new computing node to their Main AI to alter its programing and give themselves a chance. They failed, but the plans managed somehow to be sent into the next cycle.

Going on the basis that everything that happened was real and not some phantasm(i.e. dream, indoctrination, etc.) this is what I see to be the ending...
Shepard makes it to the Citadel, barely alive. Shepard faces down the Illusive Man and gets him out of the way one way or another. Depending on your choices, you get one last talk with Anderson. Then of course, Hackett calls you and says nothing is happening. But, something is, the Catalyst is already altered by the hardware programming in the Crucible. I mean hardware programming as in no matter what you program into to it, the alighnment and archetecture of the circuits creates a program with in it.
The Catalys knows Shepard is still alive and brings Shepard to it. Reaching into Shepards mind, it extracts a rough image of child Shepard has been haunted by since leaving Earth. Knowing it's plan will no longer work, the Catalyst offers 3 choices to our severely weary and tired hero/heroine. Merge with all machines, take over all Reapers, or destroy all synthetics in the galaxy. But regardless of the choice made, the Citadel and the Relays will all be destroyed.
For the green ending of course, Shepard is absorbed into the Crucible's energy stream, causing the synthesis ending, leaving no body behind.
For the blue ending, Shepard's mind is absorbed into the system, destroying the body in the process, but ordering the Reapers to stop harvesting.
For the red ending, Shepard blows out the tube, sending out the wave that destroys all synthetics in the galaxy. However, at the last moment, for what ever reason, the Citadel/Catalyst, using the beam system that brought Shepard to the Citadel, uses the same system to send Shepard back to Earth. Thus we have the clip of(supposedly at least) Shepard taking a breath on Earth once again.
Now as for the Normandy racing away, take this thought in...Joker has seen what happens when a Relay blows, so, naturally, he kicks in the FTL drive and RUNS!!! Undoubtedly, he makes a quick pick up of the team members in their shuttle( before or as the Relay explosion wave comes){I admit that last part is a real stretch of logic, but it does fit with who Joker is and his skill as a pilot}
Racing away from the detonation wave as fast as the Normandy can, being the fastest ship there is, Joker makes a good run, but eventually is overhauled by the wave. They crash land on the nearest habitable planet possible.
Now some have said what about all those ships from the Fleet still at Earth. I think those people are forgeting that all ships have FTL drives, it's just the Relays get you across the galaxy in a few weeks( or shorter) instead of the years it would take with their FTL drive. Remeber, the Relays were only used for getting between major hubs(i.e. major crosss streets, like a bus system) then hoof it from there with FTL drives to whatever solar system you wanted in the area.
And for the `Stargazer' talking with the child(I think a girl) are probably the decendants of the survivors of the Normandy crash. No telling how many survived the crash, but I would guess probably most if not all of them survived the crach, Seeing how Joker with his brittle bones was still in one piece.
Now having said all that, even though I can peice together the story logic of it all, I have to say this...(in a very low voice) I DON'T LIKE THESE PITIFUL FEW CHOICES, ESPECIALLY WHEN WE WERE GIVEN THE PROMISE THAT OUR ACTIONS AND CHOICES IN THE GAME WOULD REFLECT IN THE END OF THE GAME.
I do appreciate that Bioware is rethinking the ending of the game, and not to jog there elbow too much, but I would like at least ONE ending where Shepard and the LI re-unite, even if its just a death scene( though I would prefer a happy ending possiblity more) with an epilogue of what happened to all the team members, the major characters you ran into(i.e. Primarch Victus, Wrex-if alive in ME3, etc.), and the events 6 months after the final battle at Earth(new homeworld for the Asari, Elcor, Volus, Krogans, and Turians maybe). And scrub out the Normandy running for the hills and the desruction of the Mass Relays, though I think the Citadel should still be blown up, to get rid of the Catalyst once and for all.
