I was on Sur'Kesh and had entered the elevator to go down to meet with the female Krogan and Mordin. The moment I entered the elevator the game froze and I got a pop up on the screen stating that the game could not read the disc that I had to restart the game.
I hit okay and then was brought back to the 360 dashboard. I had to start the game up again and wait through the horribly long checks to get to the main menu of the game. I then started the game up again and had to start over on Sur'Kesh after the landing scene with Wrex. I went to the elevator again and was able to go down this time.
This is the first time I've had a disc read error from this game. This is on disc 1 of the game. The discs are NOT scratched, they are NOT damaged in any way shape or form. In fact I have the game installed on my HD so that the discs do not spin or get damaged.
I know there are others who've posted issues with their discs and have had issues during the disc swapping prompt. I have not had that issue yet. This is the first issue I've had with the discs of these games. Not including the snow effect on the black screen during load up after starting the game from the 360 dashboard.

Interesting...This EXACT thing happened under the exact same circumstances to me today.
