This is a bug I've noticed in both of my SP playthroughs of ME3:

In certain cutscenes with Kaidan the camera just focuses on a wall or something instead of showing anyone even when they are talking.  I've noticed this with Kaidan in 3 specific cutscenes.
1. The Udina coup - when people are leaving the scene it doesn't show Kaidan it just shows an empty space and then goes back to the other character.
2. When you are talking to Kaidan on the Normandy after the Gellix mission. In part of the scene it cuts to an empty wall instead of showing Kaidan or Shepard in the scene.
3. On Thessia - anytime Kaidan is supposed to be talking it just shows the scenery and not him, although you see Shepard and Liara talking.

I've had the other PC bugs too of:
Resuming a game and getting default FemShep.
No banter between Gabby and Ken at all (guess cause I like Kaidan ...)
Didn't romance Kelly Chambers - but I don't even get the mention of her being killed after the citadel coup.
Get stuck talking to Edi in the cockpit sometimes and have to reload.
Fell through a gap on one of the missions on Rannoch, where you are disabling AA guns and had to restart.
