Yup, all my bonus powers before the corruption on my first playthrough are gone. Just noticed it when I started my Insanity play through. Only had like five. Why would you tie it to the settings save file?

Posting this here too:

I wrote (the below) to Thomas Abram (one of the team) explaining what exactly happens when you start a new game or continue after inadvertantly losing the settings through a crash or something.

The kill and melee counters work whether you have accomplished them before or not.

Working Correctly

Recruit - Kill 250 enemies
Soldier - Kill 1000 enemies
Veteran - Kill 5000 enemies

Bruiser - Kill 100 enemies with melee attacks
**Striker - Kill 25 enemies with melee attacks

**Striker isn't a trophy but it is listed in the Accomplishments section (under the Extras Menu).

All the above work as expected (with a new profile).

Not Working Correctly

All other trophies that have a counter are locked whether you have accomplished them or not.

Combined Arms - Perform any combination of 50 biotic combos or tech bursts
Focused - Evolve any of your powers to rank 6
Overload Specialist - Overload the shields of 100 enemies
Sky High - Lift 100 enemies off the ground with powers
Pyromaniac - Set 100 enemies on fire with powers
Mail Slot - Kill 10 guardians with headshots from the front while their shields are raised

Tour of Duty - Finish all multiplayer maps or all N7 missions in single-player
Always Prepared - Obtain two non-customizable suits of armor
Tourist - Complete one multiplayer match or two N7 missions
Explorer - Complete three multiplayer matches or five N7 missions
*Gunsmith - Upgrade any weapon to level 10
**Almost There - Reach level 15 in multiplayer or level 50 in single-player
** Peak Condition - Reach level 20 in multiplayer or level 60 in single-player
Unwavering - Finish all multiplayer maps on Gold or all single-player missions on Insanity

** Almost There and Peak Condition defaults to Level 1 even when using an Import save file.

If the profile settings have been erased unintentionally as was in my case through a system crash, then starting with a new profile should reset and unlock all accomplishment/trophy counters.
But this isn't happening.

in addition

If you are starting a New Game + (importing ME3 save - after a profile reset) and you were on Level 49 as an example - The Almost There and Peak Condition accomplishments default to Level 1 and are locked aswell.

The only way it will work correctly is with if you start a new game with a another PSN ID (I have two which is how I know).
On my other account everything works as one would expect.

Point to note:

The difficulty is tied to your save file so if you were in the middle of an Insanity run and you were to finish it (after the profile reset) you'll get the Trophy.
The Unwavering Trophy glitched for me, and the final counter read 6/27 instead of 27/27.

In Summary:

Expected behavior:

Starting a new game on ME2 (with new profile).
All trophies (accessible from the Extras/Captains Cabin) are darkened and all counters work

Not expected/Not working correctly:

Starting a new game on ME3 (with new profile)
All accomplishments you've acquired through previous playthroughs are lit up
Some counters work, others don't irrespective of whether you've earnt them before.
See above list.

Edited title also to reflect issue.

You can add me to this list. My games froze 4 times in the last 20 hours, resetting all my achievements. I just started my insanity run, so I wasn't too far into it, but if it's bugged to where I will not be able to get the trophy, what incentive do I have to even do it?

The game is by far the most problematic/buggy game I've ever played. I've got to be over 20 hard resets now.

I hope EA/Bioware have trouble looking at themselves in the mirror, but I doubt that's true when they're looking at their new gold watches, diamond rings, and so on. While those of us that spent what little discretionary money we have in support of EA and Bioware have a game that shouldn't have even been released. They should have at least pushed back the release until these problems got fixed. There's no way they can't say they didn't know about these problems either, because anyone that would just play the game will notice them.

Shame on you EA/Bioware.
