5. The Silly Dreams:
Oh, god.
Yeah, those were the only things (aside from the end) that I had a serious "What the hell is this?" look on my face while I was playing through them---the other things you mention (opening, Crucible, Citadel's giant leap across space) I noticed afterward as I was going through the whole mess to try and make sense out of it.
One, you're right. Why in the world is (s)he all of a sudden having premonitions involving a child (s)he speaks to for all of ten seconds when (s)he had to leave Kaidan/Ashley to die? Why couldn't one of them be the subject of the dreams exactly?
Two, I get what they were trying to do---horrors of war, seeing a life cut short, haunted soldier, etc.---but this is a person who's watched people melted into paste right in front of him/her (on the Collector Base).
And three, would a renegade Shep care? I mean, not to make him/her too callous, but that sort of personality just accepts that casualties are a given. Would it really bother him/her?
