I have several accomplishments that I believe I have completed, but show up as blank on my computer. In particular, I'm 100% certain that I've completed "Lost and Found" at least 3 times, but every time I log back in, it says 2/10. I can go find stuff and it will count back up to 10/10 and then say "Complete", but the next time I launch ME3 its back to 2/10. I started with an ME2 save and have been playing through 1 campaign using a sequence of linear save games and never going backwards through my saves. All I ever do is open it up and play campaign or MP.
I've also gotten "Paramour" and seen the notification pop up in-game, yet its still dark, even though I have an active romance with Liara.
I also get "Always Prepared" every time I purchase a weapon or mod, yet it fails to save.

maybe your Options > Online > Upload Gameplay Feedback, is set to Off instead of ON which has been the case for alot of people. the only way i was able to fix a ton of my problems was to install a clean operating system to a new drive, namely my sweet little solid state drive. 
Some ports could be getting you screwed over too, since I was told an immense amount of udp and tcp port ranges and single ports, to enter into the router firewall. add me ad a friend named Metamorphisis and i'll accept and talk to you if you didn't get this fixed yet. ok?
