When I played DA:O I ended up missing a hell of a load of questsby rushing through areas, (and actually missed out on some party members as well.) The first time I played through DA2 I was a bit more careful, but I think I still missed out on things.
THIS TIME though, I am determined to experience all the content, and so I found these links on the wiki;

If I just make sure that I complete all of these, will I not miss anything? Also, I see that it lists a certain act next to each item; does that mean that that quest will always be available as long as I complete it during that act - or are there triggers during each act that may lock out some quests?
Also, if it is the former; is it very clear when advancing each act, (as I said, I played through DA2 quite quickly, so probably didn't notice.)

The acts mean the quests are only available before certain quests.
Act 1 is the deep roads expedition, so don't talk to Bartrand once you have the 50 soverigns and the map Anders gives you until you complete all of the quests.
Act 2 is the Qunari invasion. After if you choose to help Isabela get the relic, Aveline needs you to go to the Qunari impound with you to get some fugitives, don't enter the compound at that point unless you finished all the quests.
And Act 3 is the beginning of the Mage vs Templar War. This one's pretty easy to avoid, just don't accept the quest "The Last Straw" on your desk until you finished every quest otherwise you can't go to the Gallows without starting The Last Straw

You can't get all quests in a single playthrough. Many quests are only available with certain DAO imports.
