I have been playing most of a new game+ on Insanity without any bugged mission, however, two days ago, I reached the N7 mission called "Communication Hub" and I encountered a problem: after selecting the squad's weapons, the loading screen (with the blue shuttle moving) starts, then after ~10secs the sounds stops, the "loading" icon keeps spinning, the shuttle keeps moving and the mission never starts.
I tried many times and twice a message from the PS3 showed "There has been a disc read error" forcing me to quit the game, which led to a restart of the PS3 that does not like that.
I did not have this problem the first time I played.
I checked the hard drive for errors and fixed them.
