 I've came across a very bothersome issue that prevents me from beating the game. While I'm attempting to clear out the children in the Eastern Hall... Captian Garevel comes in with icon above his head (for an active 'quest') but it then disappears.  Although it stays on the toggle map (the icon for the active 'quest') but in the place of a solider that dies just moments after you enter that area... EVERY time.  This prevents me from continuing over to the main gates again to trigger the cut scene with Varel. I can see him battling the Darkspawn but I can't attack them at all nor can I talk to Varel. Also, I can't go back into the Main Hall either to talk to the merchants. 

I've tried starting from several different save points and different tactics to prevent that guy from dying, I've even tried different people for my active team.  Nothing seems to be working... 

What should I do? Am I simpling missing something?

Your basic symptoms sound like a corrupted install or a problem caused by a mod.

I did read them, and I checked those. It still didn't fix itself.
