ME1: Ashley/Kaidan. Their banter was just too funny. I also knew I wouldn't have Ashley forever, so I spent as much time as possible with her.
ME2: Garrus. The other squadie was more variable, but I usually defaulted to Mordin.
ME3: Garrus/Kaidan. This combo worked very well with my playstyle, but I also felt like I was going into hell with my best friends that I had since the very beginning.

To elaborate; he feels like my wing man, my war buddy, my closest bro... His dialog is just snarky enough that it doesn't seem forced and he usually gives level headed dialog. 

Garrus except when playing Insanity.

ME 1: Garrus (second teammate is usually Tali but sometimes I like to switch her for Liara or Wrex. For some strange reason I never liked Ash or Kaidan so I avoid them as much as I can)
ME 2: Garrus (I always go for the Archangel quest first so I can have him "Right behind me". The only exception is Suicide Mission where I always pick Garrus to be leader of second team. That's how I know my other teammates are in good hands. And he safely brings all of them back alive to the Normandy)
ME 3: Garrus (If I have a choice then Javik is my second option)

All around, it ended up being Garrus used the most. I never took one squadmate with me on every single possible mission. Sometimes I would change it up to try different scenarios. But I mostly used Garrus. His AI worked really well.

Always try to rotate them, sometimes based on dialogue options.

But I DID use Miranda a lot in ME2 cause warp + overload = win!

I usually pick based first on story and then second on the skills I need. For example, on the Geth missions in ME3, I took EDI along. On the Asari Homeworld, I took Liara and Javik.

In terms of squad members I liked, Mordin/Legion had some of the most interesting lines in the game. Tali and Garrus have the funniest lines.

ME1 was ALWAYS Ashley and Liara.  Liara for her biotic abilities, Ashley because I really liked her character and dialogue.  She reminded me of a lot of the women I served with in the military.

ME2 was always Mordin, plus a randomly chosen other character.  Mordin wasn't a great fighter, but I loved that character too.

ME3 was basically random, but I only took EDI once.  Didn't see any point to it, since she'll still communicate via radio if you didn't take her.  What a waste of a character slot.
