I would have died fighting the Revenant in Redcliff Castle . ( that is because i had leliana and i didn't know that i could open the gate and let the 4 knights help me )
IF DAO was "real life" and you could only die ONCE...how far would you have gotten in the game?

I will die when i go to search for the darkspawn blood, i took me a lot of time, i died a lot ....=_= almost give up the game, but thank god i didn't.

My shield and sword warrior encountered death for the first time when he faced the Revenant in the Circe Tower.
Had to reload once, playing on normal.
My party members would probably have died earlier on in the game, though. But after all, tanks are there to last.

I don't remember if I managed to kill Ishal Ogre in first try. Anyway if he didn't kill me, then it was those hungry refugees in Lothering. That really pissed me off.

I would have been killed by the Revenant in the Magi Tower, I think. I don't exactly remember if it was my first death in the game (I'm on my first playtrough as a Dalish Elf). If I started as a human noble I think I would have died much earlier. My wife started as a Human noble and those fights in the Castle were harder than the ones I had in the forest or in the ruins.
But that first Ogre is a pain in the %$$##@#, que almost got me. When the fight was over I was the only one standing and almost dead. I think I was lucky and if it was real life I wouldn't resist those nasty wounds.
Editing: I've just remembered. I was killed by the first major Darkspawn group in Karkari Wilds. Sorry :innocent:

in my first playthrough in Hard Mode with a fighter i first died from that girl wielding the "summer sword" , you know Loghain's friend , dont want to say more and maybe spoil the wheres or how for early stage players

one of the toughest battles for me until i made use of Morrigans all four earthquake firestorm tempest icestorm spells however this girl was still standing !!!!
i think though that by fighting i missed a large part of the game i would discover if i had chosen otherwise....

And the there was the visual plugin to the tv. Epic fail number two.
And then I started a game and later created an Xobx live account and found out that I couldn't access the downloadable content on my previous account because it couldn't go online, meaning that I don't drop off my overflowing gear at Warden's Keep and had to restart. Epic fail three.
As far as dieing in battle, if you count the entire party being dead as dieing (since obviously no one gets revived from being "unconcious") then I can at least say that it was when I felt like fighting a dragon on a whim and didn't even try to prepare for it properly before hand. I got through 2/3 of her health fine before 1 fire breath killed my tank and backup tank (full health of 0 in 1.5 secs). After that I at least got it down to 1/4 health before I and my healer were killed off.

An interesting question by the OP. I would have to say though, that its a rather difficult question to answer truthfully. Reason being there are plenty of sticky situations and places (especially if you throw into the arena what difficulty you start on, myself hard+nightmare) that can get you razzled. More importantly I think that majority of gamers will just go head on into certain battles and areas not totally thinking strategy because they know they can hit esc load. Now if they implemented a hardcore mode and said the main PC can never die, and you try this on hard or nightmare, the player's train of thought totally changes. You're first line of thinking becomes how to keep the main PC alive and best way to handle the situation. I think that would actually be a fun add on - HC mode. No deaths period for main PC, if you die you can't reload.

Well for starters, I wouldn't have let Iona open the door...so that likely would have been me getting ambushed and shot. Though I suspect that I wouldn't have been stupid enough to just fling the door open with the attitude of "shoot my naked chest".
But combat is a lot of luck IRL. Sometimes I fight lesser fighters and they surprise me with a move I didn't think they knew. Sometimes it's that way with me against better fighters.
Really though, combat is about deceit.

No, likely, I would have told Iona to hide cause damn, I don't want to be caught in the act by anyone really. Gotten dressed and listened to the door a bit before deciding what to do. I got good ear to make up for my bad eyesight and if fighter-practices and sparring sessions are any indication I have pretty good battlefield awareness as well.
Still 2 against 1 on opening the door isn't very good odds, but a doorway is a chokepoint so it's more like 1v1 two times. However, one of the guards has a bow...means I must be aggressive. Use front guard as cover while killing him, then dispatching a man holding bow while dual wielding is a piece of cake...it all depends on how lucky I am with that first guy...and how much my dog helps.
I likely would have died in one of the other castle fights where I got ambushed. I'm good at ambushing...but not very good at surviving them as my mass combat skills are lacking. I'm better for small scale fights and 1 on 1. If mom and doggy went down, I would run and hide while picking off lone soldiers or soldier pairs as I made my way out of there.
Would have told Gilmore to lead a fighting retreat from the main hall as the castle has effectively become linear with the blocked off passages. Main hall lost its significance as soon as that happened. I would clear and evacuate the bedroom half of the castle, then begin the fighting retreat as we all escaped through the servants quarters. With more men, we would have been able to drag dear old dad with us. Not to mention shove a healing poultice on him.
Though if he were really my dad, he would rig up some McGuyver styled trap for whomever came next.
