Garrus - I take him on every mission. He is a bro.
Legion - I love legion. Gonna miss the poor bastard.
Zaeed - He is an amazing warrior and has the experience my crew needs. Plus, awesome stories.
Thane - Who knows then I may need someone assassinated?
Wrex- Again, he has experience and is a great leader.
Grunt - Another Krogan couldn't hurt. Plus, he will kill whatever I tell him too.
Miranda - Powerful biotic and intelligent. Capable of making quick battle field decisions. Her knowledge of Cerberus might be useful.

On the ship but not fighting:
Mordin - Do I even need to say why? The man is a genius and a doctor. He is too valuable to risk in combat.
Liara - Again, the Shadow Broker is incredibly useful and risking her in combat would be idiotic.
EDI - She is the ship.

Kaidan: For his awesome overload skillz that saved me more times than not on Insanity
Zaeed: Nuff said.
Kasumi: Her cloaking is amazing.

Is it bad that I never used Mordin or Samara unless I had to? were they any good?

Mordin is actually pretty good at burning/freezing ppl, so he's good against Bio/armor units, and Samura is just nasty with her Area Pull/lift ability, instant target practice.

Also, its always nice to have a Asari to come with you on any mission, since charming ppl will get MUCH easier with Samara's outfit:whistle:

Miranda- Her skillset is excellent plus shes my #1 romance
Garrus- Bromance
Wrex- Its Wrex..
Liara- Singularity is useful and she would be in my "no cheating" playthrough
Zaeed- One of the biggest pricks but hes wonderful when it comes to dishing out raw firepower
Javik- Don't got him but he seems to have some funny shiz to say so hes in. Plus hes a Prothean so why the hell not.

 Tali:  My favorite LI.  Best romance in the game IMHO.
Garrus:  He's like the brother and wingman to my canon Shepard.
Wrex:  Zen Master of Badassness.  Has my vote for Galactic King!
Javik:  Has truly a wicked sense of humor. Has my vote for Galactic Prince (He's so much like Vegeta)!
Legion:  Best robot character I've seen.
Mordin:  Hiliarious Model Scientist Salarian.
EDI:  Fun character.  SR2 wouldn't be the same without her.

Runner-Ups
Samara: Mass Effect Jedi or Space Paladin
Grunt:  Dat Meatshield.
Jack:  Great character growth in ME3.

I felt the choice of squaddies in ME3 was a good one, with former squaddies being satisfyingly "otherwise occupied". The focus on Earth made having 2 humie squaddies interesting. So:

Garrus- Good Turian Soldier, bro
Miranda: MShep Li, Tech/Biotic, Ties into Cerberus
Jack- Pure Biotic, interaction with Miranda could cause for some fun moments, unfinished business with Cerberus
Tali- Engineer, one of those who were with Shep from the start, can be switched out due to Admiral status
James: another soldier but there for some comedy (possibly replaced by Javik or Grunt)
Thane: Infiltrator Biotic. May change him out, depends.
Ashley or Liara depending on plot.

Possibly Liara as the Shadow Broker but on ship the entire time, except for a mission of two.
On Ship for a time:
Wrex: leads the Krogan
Legion: leads the geth
Tali: If not a squad member
