I'm just going to post here what I tried to get help with on Twitter and go from there.

@masseffect @bioware @Playstation @BioEvilChris Just finished my 2nd Insanity playthrough and still sitting at 25/27 for the Unwavering trophy. What gives? I did everything, all priority missions, N7 missions and side missions required for the trophy. Got the Insanity trophy on the 1st playthrough.I followed a guide for the 2nd playthrough to ensure I missed nothing. At a loss here what to do. I want this Platinum!! As a side note the more I play Insanity the easier it gets. I've had MP freezes resulting in file restores so not sure if that could have caused the issue. Please reply and a referral to EA support will just get me sent to the bioware community forums for PS3 support, which will go unanswered. I can send save data but not the profile data.
If its at all possible to speak directly to someone with a technical background I would appreciate it. #ME3 #PS3

About to go for a 3rd playthrough, only trophies left are Unwavering and Overload Specialist and the Elite of course. I'm lost really and don't know what to do to resolve this.Platinum'd ME2 and this will just be a huge letdown if this keeps me from doing the same in ME3.

Yes the freeze reset the Accomplishments. Some are locked out too

I didn't get the Unwavering Trophy either.

I contacted EA Support and they of course threw it back to these very forums and to the Devs, so it seems.I'll post the email:

I thanked him for the reply and am hopeful but...if these very forums are what EA Support considers our helpline and a "BioWare Edmonton Community guy" considers EASupport our helpline the loop continues.
I'm really at a loss as to what to do from here on out, its an issue, known imho, but playing "pong" with our issues and the silent game doesn't seem to me to be a very effective way of letting your loyal, which I am and continue to be, supporters of Biowares games know that they actually mean something and will be adressed. I'm not looking for a full on technical thread about .trp files, save data, and Profile save data but ANYTHING at all would be greatly appreciated. I love this game, as I did ME2, and I worked my arse off on it.Only to have MP screw me out of finishing up the Platinum. Now I'm stuck @89% with noone to contact for help..

Thought the term "trophy" was a dead giveaway. And isn't this the PS3 Technical Support forum board? And its the time, work, put into the game on Insanity. Its a goal, not a play it on Narrative and brag that I beat ME3..

Update: Going on with my 3rd playthrough Insanity
Finished Mars
Citadel I
N7 Cerberus Lab
After doing the last one I went up to 26/27 so the counter is going again.Still odd seeing how I did everything on the 2nd playthrough for the Insanity trophy..but about to do Palaven. I'm sure the last one is a side mission because I already have the Insanity trophy.Really I'm clueless why its giving me credit now, I have a theory that I'll mention if my next side mission gets the 27th for the unlock...

Same here. Game crashed while I was playing multiplayer one day corrupting setting file and all my acomplishments were gone. Didnt get the unwavering trophy and the one for 5 000 kills... They sent me from EA support here, but I see there's more people like me and more post without any answer from Bioware. I'm not going to go through a whole game again just to get that trophy, ME3's improved cover system is too frustrating for me. I died way to many times because Shepard couldnt run trough cover but sat on his ass:]

26/27 locked out again, nothing left to redo mission wise. Hell if I know whats going on..

Hmm... That sucks

I've gone back to ME2 to redo my Renegade run with the correct choices this time.

Hopefully by the time I start on ME3 again - some of the bugs including the profile one are fixed.
I want all the counters to work properly. not just the kills and melee ones.

Same.. I had 4500+ kills for the veteran trophy and then my PS3 froze and I was reset to 0...

Ok got the Unwavering trophy finally. Funny enough I got it on my 3rd playthrough from doing Priority-Palaven..which I had already done on my 2nd playthrough getting the Insanity trophy. I had, actually, changing the difficulty during Grissom Academy(Hijacker trophy the easy way) on my Insanity run (2nd Playthrough) My Profile was backed up to the cloud and had a save before the Academy.Got the hijacker trophy, d/l'd the backed up profile/save and resumed my Insanity run.Not entirely sure but it seems you can lock yourself out changing the difficulty at all for Unwavering. Which is way it started counting again on the 3rd playthrough but I don't see how it counted Palaven twice..Anyway, got the trophy and just Overload Specialist to go for the Platinum.Lots of work, frustration and head scracthing but fun and worth it in the end. After all this time Insanity mode is cake now, I actually find it easier than ME2 on Insanity. Hope this helps anyone having problems with Unwavering. ~Fin
