If you choose to save Morinth in Samara's loyalty mission, and then you romance her and she kills you, do you get the Insanity achievement?

You get the achievement by completing the game. Morinth is completely unrelated to it. And no, getting killed by Morinth doesn't count as "completing the game".

You get the achiement by Having sheppard survive the Collector base

I've never had Morinth kill me, but wouldn't you just get the 'critical mission failure' like any other gameplay death and then restart from you last save?

Yeah. Getting killed doesn't count as "winning".
