-Fate of the Rachni queen.
-Ashley or Kaidan
-Whether or not to sabotage the genophage cure. I spent several minutes debating that one, and ultimately decided not to. The krogan were too valuable an asset for me to go behind their backs like that.
-Quarians Vs. Geth. Another hard choice. I was relieved that I was able to make peace between them.

This was really tough for me, since my first playthrough it was Wreeve in charge. In the end, Eve being alive convinced me to let Mordin do it.

The second ME game it was should I destroy/brain wash the geth that sided with saren, again something that I thought would effect the outcome of ME 3 and in a way it kinda did but still was hard to make this call for other reason...
The third ME game I would have to say it was the Salarians or Krogan, with Wrexs and Eve in charge it was not hard at all but with Wreav it was....

Toughest decisions in the trilogy? Hmm....
ME1 - Whether or not to save the Destiny Ascension. As the first human Spetre, I felt a strong sense of duty to the council. They had taken a chance on me and I didn't want to let them down. But on the other hand, I didn't want to take a chance on having Sovereign succeed. Ended up saving the council. It worked out alright.
ME2 - Rewriting or destroying the geth. I dilly-dallied for as long as I could on that decision. I felt like making it put me on level with the Reapers. Come in and either destroy or control that which opposes me. I ended up rewriting them because Legion's consensus was ever-so-slightly in favor of that. I figured if they were upset with it when they found out, maybe I could make it up to them later, but there would be no chance to repair bridges with the dead.
ME3 - Whether or not to tell Wrex about the Salarian deal. The Krogans could easily be nasty business for all other races after the conflict with the Reapers gets finished, and sooner or later, Wrex is going to kick the bucket. Plus, he's all for the advancement of his species in a big way. I had hoped to talk to Mordin about it privately, but ended up caving the second time Eve asked me what was on my mind. I decided I trusted her, if not Wrex, and hoped for the best. Besides, I didn't want to risk losing Krogan support if/when Wrex found out I had betrayed him... and this was before I knew that if you do, he does find out.
The one in ME3 definitely took the cake, though. I thought most of the other decisions were pretty clear-cut with the way I was playing.

ME 2 - Though neither this nor ME 3 had decisions as difficult as ME 1's, Legion's loyalty mission was a highlight for me.
ME 3 - Whether or not to cure the genophage. I was all for it until Wrex started talking about expansion. Then I had to think about it for a bit.

Same prediciment as you, but I concluded the Geth were innocent and the Quarians started it...sorta. Sad about losing Tali and Koris..

The Virmire Survivor was always a "whatever" kind of choice for me. I actually disliked both characters, but I disliked Ashley more. Good riddance. I told Kaidan to just stay in the hospital. Screw those two.
What really got me was actually whether to immediately go to save the crew from the Collectors or not. I still had some stuff to do, but I didn't want Kelly and Dr. Chakwas to die. In the end, I put it off for my first playthrough. If I had known to plan ahead, that wouldn't have been an issue.
Of the choices that you HAVE to make?
Whether to kill the Rachni Queen. I had Wrex with me, and he makes a very valid point about them. But in the end, I couldn't make myself kill off an entire species. Even in that situation. Didn't realize it wouldn't mean anything later.

Another hard decision was between rewriting or destroying the heretics. Both options seemed immoral. It took me awhile to decide.

Kaidan/Ashley? Killing Wrex? Quarian/Geth? Or maybe something smaller?
Mass Effect was loved by me because on numerous occasions I just had to take a break and think it over.
What is more important? My friend or the fate of the Galaxy? Is this decision worth the loss?
I, personally, had two: Wrex on Virmire and Geth/Quarian decision (I did not have the option for both to survive).
I play paragon almost exclusively, so a few of the more ambiguous paragon/renegade choices were really hard to make.
Still, one of the interrupts really stood out in Mass Effect 2. On the Dossier: Archangel mission, when you have the opportunity to weaken the air support. I took the interrupt to see what would happen, and was... pretty damned mortified. I hadn't saved for a while so I had to restart the mission, but I couldn't commit murder.
