reapers never saw it coming.
Oddly enough, though i typically choose squadmates over anything else, I went with the queen on this one. And guess what? Grunt still lives
WHAAAT??? HOW?!?!?! What options did you chose?
He has to have been loyal in ME2.
he was loyal in ME2 =/ I did his stupid puberty quest and killed the only thresher maw in the game. Why did mine still die, I wonder?

Thinking about it though, Krogan are too violent to be a space faring species. They ruined their own planet with thermonuclear warfare. Essentially I could never let them live unless I had the ability to fully remove all abilities for them to leave their solar system.

ME2: Heretic decision, and saving Maelon's Data? Decision between potentially saving more lives at the cost of your morallity by using these blood-stained data?
ME3: Quarian Geth, they're both right and WRONG at the same time...

For me it was the Rachni. I put the controller down and walked around the house for about 10 minutes. I figured it would have huge consequences in the conclusion to the trilogy. Heh.
^this
except I'm on PC

The toughest choice I've had to make is if I wanted to play pure Renegade. I decided I couldn't because I know I wouldn't be able to shoot Mordin in the last game. Or shoot Wrex. Basically, the Tuchanka mission/Genophage decided that I will probably always play Paragon.
This has been a big one for me, too. I only have one playthrough on all 3 games and was intending to play a (more) renegade character on my second run... but I just don't know if I can do it knowing what I'll lose... and that's not even taking into account whatever's going on with the end.
The quarian/geth conflict was really a no-brainer for me. I was so pissed at the quarians that I was ready to watch their whole race burn. I would have been really upset about losing Tali, though.

Toughest decision. Trying to decide whether or not to play ME3 again.
What did you decide?
I chose "no".
LOL. I can't say I blame you. I decided to wait and see what they announce. I sense it will be multiplayer horse**** and/or non-ending related content. If so, the "no" decision is looking more and more appropriate.

Also the Rachnii queen. Trust her and have a possible ally and not kill off a species? or not trust her and save thousands from a possible enemy. i remember squealing in ME2 when the Asari gives you the message that she will join you in the fight against the reapers in ME3........Yeah, that choice REALLY mattered......

Bring down the sky on whether or not to save the hostages or kill the terrorist. I honestly sat there wondering "well if i kill him now, yeah 10 people die but that saves 100 people later. But if i save these people now, maybe later i can catch him before he does any damage" Same with Zaeed. It was stupid DLC that had nothing to do with the "main" plot line and i think i spent a good total of 20 min attempting to figure it out

Yeah man, this. I thought there was gonna be rachni soldiers all over the joint cause I saved them. But nah. This choice was really difficult but now I may as well put them out of their misery before they see the ending.

ME2: Rewrite/Destroy the heretics, and the final decision in Overlord. I only let Dr. Archer keep David once, and today, I found out what happens in ME3 when you do. I wish I had the option to kill Dr. Archer on the spot.

I felt really bad when I had to shoot him and then watching him trying to cure the genophage. He almost made it.
