Those failures were caused by the Vanguard glitch. Every time.
Everytime I've encountered the deadly glitch, I was leading the team to what looked like victory. But each time, the vanguard glitch stopped me short, leaving the game down to three people. It's not easy to stop either, because when on the verge of death, a vanguard always has the urge to charge. And if a spurt of lag hits, the glitch engages. We then go on to lose the match. Even if I left, it wouldn't matter, as the difficulty wouldn't change, and I would be cheated of the credits and exp that myself and the team earned.
There has to be a way to fix this GBG relating to the Vanguard class, which was the second most played class online in the stats posted by BioWare. Save the vanguard.
Out of 5 games, 4 ended in failure. (Silver Vanguard Glitch)

This has happen to me twice tonight. We were doing so great then all of a sudden i start flying in the air or falling though the ground. Its really annoying.
