I can confirm 5 of those already listed have happened to me and one extra.

1 = Ranged Banshee attacks have from time to time flown through walls. Mainly when curving around corners.

2 = When whacked from behind I stagger backwards into the enemy. Always happens with phantoms. Actually the main reason I fail most of my mission. I am whacked in the back by a phantom and by the time I recover from the melee she whacks me to death.

3 = Sometimes I can't climb up the ladder on Firebase White.

4 = Can use Survival Ops when health and shields are full.

5 = Rockets do pass through Atlas's sometimes. I believe this mainly happens when you are tapping "fire" while your character is still pulling out the gun. Then when you have eventually pulled it out and you're still tapping "fire" your missile passes straight through the damn Atlas.

Extra = And the most annoying one, when you have a tiny. tiny bit of health left and then use a Survival Ops pack to recharge, if you are shot within 1 second of using it you die anyway, even though your shields and health are full. sometimes this uses up all of your Ops packs.

Sometimes when you die with a Cobra in your hand, maybe just when you're about to use it (don't think it makes a difference, died before using it), when you revive yourself and try to fire it, NOTHING happens. Tried firing it multiple times when I got up once, just to see if it had a long delay between firing but nothing happened. You have to unequip it and equip it again for it to function properly. This can happen at crucial moments and can often lead to mission failure.

Rocket trooper rockets occ go through walls pm if you want a video. Seems to be common amoungs all projectiles incl prime shots some times

Something like this occurs on the PC as well. Using a Krogan Sentinel, I've seen it multiple times. However I don't pistol whip, I headbutt.

However I've been told by other players that my melee looked normal to them, only I was seeing the animation incorrectly. It also fixed itself, I assumed, but I probably fixed it by taking cover again.

I can confirm this on PC. Happens a lot on White when Geth R.T.'s are coming down the stairs. Clips through the walls very often there.

Occasional bug where projectile powers (Incinerate, Warp, Cryo blast, Throw e.c.t) don't do anything. You can see them travel to the target but they dont have any effect when they reach it (PC)

Hey, first appreciate you actually going through and organizing all these, gold star.  

That being said, I do think that the lag-related issues should be included as they are (and should be) accounted for/fixed with any proper netcode. Specifically, for things like Spec Packs/OHKs, it should probably respect the values of the client when there's a conflict. It just seems unfair otherwise. Other things like the oh-so-common invisible enemies/weapon blanks (on both projectile and even hitscan weapons) is also code-fixable - there either needs to be some checksumming or sanity checks built in. Same for players/enemies through levels. Even a periodic sanity check (once every 10 seconds) w/ some really loose positional bounds testing would do wonders.

UPDATE: that being said, it looks like you're including most of the big ones so yeah, I guess just read that above paragraph from a programmer's perspective on what should or shouldn't be possible to fix in netcode. I'm assuming their doing something a la sending huffman-coded frames via UDP, but apparently they're cheating a fair amount on state data which makes is pretty easy for the game to get into an inconsistent state.

Can confirm this one myself, and have had others report it happening to them as well. The Krogan will simply run through the enemy, doing no damage, causing no sounds as if he hit them either, just as if you used the melee on thin air. Headbutting them (regular melee attack) still seems to work though.

Double confirmation! Mrrrrrwaaaaarrr.

Also, heave melee never triggering. Ladder climbing never triggering. Falling through the map on various maps, the most notable of which is firebase dagger.

There was a list of fixes that will be included the patch coming out next week and I did not see any MP fixes listed. Were any included in either the patch or the upcoming DLC and not included in the list?

I'm playing on the PC and have experienced this as most of the classes. I don't know what causes it or what makes it go away and the only possible "fix" (besides the one offered already) is to melee while running as that causes a Heavy Melee attack.

Very annoying.

I've also experienced the Vanguard dropping out of the map after "death charging" an enemy, and not being able to charge an enemy despite clear line of sight and a target icon. I've also had some instances of attempting to use an Ops Pack and it either won't activate, or I still die immediately after use.

Last but not least, the disappearing packs in the Store. This has happened countless times, but I'm certain I'm not just flatout losing credits. Just today I "lost" a Premium Spectre Pack but when I checked, I saw my Claymore leveled up. Just wish that when it says "Error processing.", it really is an error and doesn't deduct any credits.

 The following issue (although it is not really serious or even harmful) occured on XBox 360. When on Firebase Dagger I was instant killed by a Banshee. After it picked me up and "stabbed" me it dropped me outside of the map. When the round ended and I was automatically revived by the game I fell through the map. After about 30 seconds I respawned and everything was fine. Just a wierd occurance.

Some players have reported killing or knocking down enemies only for them to lie down as if defeated, but still shooting at the player. Unknown cause, need further information.

This happens when you do two powers really quickly or instantaneous. You have to use powers to kill them or a lot of bullets.

Anyone mention invisible Powers? like Stasis bubble, Throw, etc.?

also I'd like to reiterate the Revenant reverse stability. (w/ turian/stability bonus' and dampener)

and invincible enemies on the ground who are actually invisible killers (this was also hilarious when it was a phantom and we ended up in a 3 minute long revive circle) but the host was dead and he said he could see it so more of a lag issue?

some powers don't seem to work if an enemy is "too close".

geth hunter pops out from behind, fire 'carnage' to attempt to stagger back, doesn't do jack sh*t.

Happens with energy drain as well, always against hunters..
its like it autoaims to anyone but the cloacked enemies.. even if the cloacked enemy is right infront of you almost hugging you.
this can get your Salarian engineer killed very easily (Died so many times while reloading my widow and because the energy drain didnt hit where I was aiming)

- Unable to Revive teammates:
How it works: Several times over the course of a night, other players and I will run into the situation of not being able to revive a downed team mate. The player will be standing directly on top of or beside the downed player, with the "Press A to Revive" prompt on screen, and attempting a revive though your character will do nothing. If cover is nearby, they will simply dive to cover. I have no idea how to remedy this in game, as every time it happens either I die trying to get it to work, or my ally bleeds out. Any more information on what triggers this, or theories as to why this happens are welcome.

I tend to find that this can be circumvented by "physically looking" down at the player who needs reviving. Takes time and leaves you even more exposed, but seems to get the job done most of the time.

Yep, multiple times.

Plus:

Can't revive glitch.
Stuck on wall glitch.
Out of body experience glitch.
Full on lock-up have to hard reboot glitch.

But not often enough to kill the fun yet!

PS3 by the way.

On 360 last night, I had both of these glitches.

My Turian soldier's Geth Pulse rifle kept disappearing on Firebase White, but it didn't seem to have anything to do with ladders.

On Firebase Reactor seemed to be where I was getting the 'play dead' bug the most, but I hadn't seen it at all before last night.

id wish to add, i had a character completely reset as if it was just deployed

the class was (and still is) at level 20, all 4 chars deployed time ago. upon opening multiplayer, i find this one character with no name, no appereance set, standard common weapons in inventory, and not a single skill point assigned but the standard first point into the trademark ability of the class. other 3 characters were just as i left them last time, all named, equipped and with all appereance and powers options on.

first time it happens to me, but not sure if it has ever been reported here. anyone?

Ok, so I've been keeping track of my gear on a spreadsheet. I can confirm that if a pack "fails to purchace", you do get items. They just don't show up as new.

I had a Premium Spectre Pack fail last night. So I looked through each of my weapons mods and characters. Turns out that I did, in fact, get two rare weapons and one weapon mod. I don't keep track of consumables, so there was probably some of those too.

I don't know if this is always the case, but maybe it isn't so bad after all. It just doesn't tell you what you got, but seems like you at least do get your loot. So not all is lost :)
