I can confirm this happens for PC as well and it happens quite often. Makes the gun nearly unuseable.

Nr.1 most annoying bug in the game and it makes playing a Vanguard almost impossible. I'm getting stuck under/above the map in almost 50% of all games I play.

Confirmed for PC, only happened to me once though. At first it said something like "There was an error with your purchase" and gave me back my credits. Then I restarted the game and suddenly my credits were gone.

There is a glitch the stops people from rolling forward. Not sure how it occurs, but it can be fixed by going into and out of cover.
Both of these are for PC.
Not sure if reload-cancelling is a glitch or a feature...

also make sure you add mine about the invisble guns that have to do with ladder animation
also add to that falling through floors on some levels when you go down and you getting reset on spawn...
also when you accept an invite it makes you go back to the main menu select a character again and all sorts if other crap!

I've only heard this on the forums and never seen myself but failed Grab attempts may glitch the enemy out making it invulnerable to damage.
Banshees sometimes lose their model for some reason, you can still shoot and damage them and there's still power reticule on them but they turn completely invisible. I've seen this happen on Firebase Reactor on PC as the Banshee casually walked past me turning visible/invisible every few seconds.
Banshee has 'vacuum hands' that can grab to instakill you even if you're already far away from her. This happens when you're not a host and your connection is somewhat laggy.

Can confirm this happening on 360. Firebase White, Starting at the top near the ladder, targeting Marauder (Shields) on the LZ, get interrupted halfway through charge by a Cannibal's melee. I was the host. Vanguards floating above/below/off the map happen on the 360 as well.
Invisible Grenades are also pretty common. You can hear the timer sounds going off but there are no indicatiors (physical grenade on the ground or the warning icon) appearing. This usually happens on or nearby staircases on the maps, most commonly on White.

Vanguards falling out of the map I can confirm on XBOX 360 as this is a highly common glitch, in fact potentially (although uncommonly) leading to the quick shield recharge glitch where everyone's shield and health is constantly recharging.
Also to expand upon the healing team mate glitch, RARELY the press A to revive will not even appear, however I have no idea currently on the circumstances, but from my limited experience with it medigel still allows the player to revive.
The most worrying of all though is the texture loading glitch, where an enemy will exist in the game but will appear to be completely invisible. This is highly common with Turrets, and I have also seen it on phantoms (and been executed by one) and a single Banshee so far.

Do you mean the error where characters hold invisible guns and run around? I will add it. Have seen it myself, however I don't know what the ladder glitch is and what you mean by stepping on teammates heads. How does that work? And further info on getting off map on Dagger and White would be useful. Thanks.

Always Shown Weapons Firing Animation: Generally appears on automatic weapons of team members even when not using the gun.
Adept Casting Glitches: On many games when using an Asari the warp, throw, and stasis animations are absent. The cast does work but it's hard to tell exactly if it was a hit or not.

There are bugged ammo boxes on some maps that do not regenerate thermal clips properly. Specifically, the ammo box near the LZ on Reactor.

medigel sometimes doesn't work when you recently used an ops survival pack
media gel doesn't give enough immunity to escape
tactical cloak does not work if your whole team is invisble or you solo a mission
sometimes if you grab an enemy while they are begining the leaping animation the enemy becomes non killable and you have to let it kill you

- Promoted characters keeping skills and ending up with -86 (or there abouts) skill points
- Unable to climb ladders, usually happens halfway into a game
- Geth rockets and banshee projectile attacks flying through walls to hit the player
- Inability to target/activate mission objectives when clouded in smoke from a Centurion or Atlas (due to the smoke disabling targeting)
- Some elaboration to kingtrouble's mention of the vanguard rez glitch - this causes the vanguard in question to constantly take damage until death. Only way to stop it is to leave the game. I've given people gold medals for reviving me so many times.
- Some low cover does not provide the ability to grab enemies over them (I'm not sure about where they all are, but one constant issue for me is the set of 'computers' on Firebase Glacier just down the stairs inside the building). Simply does not give the grab option, ever.
- Widow I + sniper rifle rail amp I: This has, in my experience, caused the damage to go down instead of up. In multiple games (this only seems to happen vs the geth), the sniper rifle amp prevents one-hit kills where I could get them before (on silver). The geth in question usually dodges to one side with one hp bar remaining. On silver games without the rail amp, body shots on geth with the Widow I are one-hit kills for me. I'm sure this will need to be confirmed.
I'll update this post when I find more repeatable/random multiple occurance bugs.

Not a bug per say, but there should be a minor cooldown on Survival Ops healthkits, like 0.5 seconds or 1 seconds. If you get staggered and you hammer the button away it's very easy to "eat" multiple healthkits without any added benefit.
Revenant has negative recoil (shots lead it downward) on Turian Soldier + Marksman (accuracy evolutions) + Stability passives + Recoil Dampener mod mk.V

Yes I get this a lot. Your weapons fire but do no damage. Power activate and seem to work (you see projectiles flying) but have no effect and do no damage. You can not activate anything (comm stations) or revive teammates, and can not get ammo from crates. When you get shot, you generally only lose your shields/barrier, then whatever what shooting you seems to stop and move on to something else until your shield regenerates.. Sometimes you can "fix" this by dying (completely, totally dead and in spectator mode).
There's also another reloading animation bug. Happened to me with the Phaeston, I believe what happened was I was reloading while I died, after being rezzed I could only fire 1 bullet, then it would go into the reloading animation... it stayed this way until I died again...

This is generally a lag issue. If you host or find a fast host it will not happen.

1. Enemy immune while suspended in midair.
I've seen this only with the Cerberus trooper so far. He's in the middle of a spawn drop and is immune to EVERYTHING. One time I was able to heavy melee him out of it but, for the most part, you have to start a new game.
2. Vanguard charge bug.
Yes, we've talked about this one nonstop. You charge right before you die and you end up glitched for the rest of the match. 1 useless player is not fun.
3. Stasis biotic explosion makes some immune.
Some of the enemies caught in the explosion become immune for a while, that shouldn't happen.
4. Invisible enemy.
Sometimes the Banshee goes invisible for a while. You can still see the hitbox though.
Geth hunter sometimes goes completely invisible, you need to do aoe moves to kill him.
5. Enemy on ground shoots at you.
It looks like he's on the ground but, he's not. He's actually walking around somewhere shooting at you. When you shoot at it, sometimes the health never goes down.

- You can use survival packs at full health/shields. I consider this to be a bug. I confirmed this today.

Hmm... I'll add this to the unconfirmed section. It's probably due to lag, but also could be due to the projectile glitch mentioned earlier. Thanks for the info.
However, it's also possible people simply miss
