2. Vanguard leaking outside the map is probably related to the physics - you can push target on the obstacle, then if Vanguard charges he could get bug.
3. There is also a bug with spawning mechanism, that perhaps enemies can't use waypoints? because are occupied by other enemy (which has been already killed by power usage and do not block the way).

I've seen this happen on firebase white/Geth/Gold as a salarian engineer. Power and missile immediate effect graphics aren't visible but powers apply normal game effects (i.e. dead/injured enemies). Decoy works but is invisible, exploding graphic appears as normal though.
Possibly related bug: when enemies die they remain standing where they died, but are otherwise dead.
Offering another report on teammates retaining resurrection circle when alive, but no action occurs when 'use' is applied when the circle is highlighted.
Offering another report on sentry turret's flamethrower sound continuing to play as client (pretty sure, i rarely host).

Stasis Bug. Mainly seen it with phantoms, don't know if they are the only ones. But Phantoms sometimes manage to simply walk out of a stasis bubble they just got stuck in.
When they get hit by damage phantoms fall to the ground then sometimes seem to get stuck in that position. But it doesn't stop them from using mind shots.

This is not solely happening to Vanguards, it's just easier to happen to them because Biotic Charge takes a little while to go off. It can actually happen to every character, if you are doing something right at the moment of death.
I was playing a Sentinel on Reactor, was sitting in cover, we were being swarmed by Husks, and there was a Ravager near the end of the room blasting us. I'm not 100% exactly what I did, but I activated a power (i think I overloaded my armor to get the husks off me), right as the Ravager killed me.
I died, but immediately my character was back sitting in cover at the same spot. I was able to fire and use powers. 3 seconds later, however, I saw the message 'Being Revived'. That in itself didn't do anything, because the round ended right as that was happening.
I had the exact same glitch as the vanguards after that. I was not on the ground, instead floating through the air in increasing steps. I was still able to fire and use powers, but eventually I was rooted on the spot and couldn't move. Finally I floated through the roof, and got killed by a Banshee that was still able to affect me.
- Another issue is that Mics don't work for a majority of people. I have a mic that works just fine in everything I use on my PC, but in ME3's MP it doesn't work. There's a ton of users with the exact same issue.
- Another issue is sometimes a critter "dies", but it doesn't really die. It lies on the ground in a death pose, but it still shows health, and it's still firing. And it's firing right through EVERYTHING at you. You can be half a level away, and the thing will still kill you. And while it's "dead" like this, it seems to have massive regeneration, because it's a royal PITA to try and kill.
- Final issue: On the heavy industrial level (forget what it's called.) if you're playing Cerb, near the far corner where you usually get a hack, Cerb troopers will jump down from that overhead beam. Except in some cases, one trooper jumps down and gets stuck about halfway, and then two troops will stand almost in one another on the beam itself. (usually a nemesis and a grunt.)
The funny thing is that people see that one guy hanging in the air, but he's basically invulnerable. You can have 4 level 20s firing at him, and he won't even lose a pip of health. You have to kill the two guys on the beam first before the bottom guy becomes killable. I've seen people quit matches over that because they think the level is glitched.

This happens to me on the PC all the time. It's happened everywhere from point blank range with no cover between the target and myself to crossing the map through cover. This also happens on my Asari vanguard, and is not really restricted to Nova/Charge spamming.

Ah yes. Forgot this one, will add it. Thank you. Also if anyone can find that screen shot that was floating around the forums of this error, please post it, and I'll add it. I need more information though, how does it affect gameplay? Do your skill still act as level 1? Do you gradually gain points into positive?

Re: Falcon Blanks - Cobra Blanks
This is partially related to Cobra blanks. Against Geth in a crowd it's too easy to get staggered and fire a cobra that does nothing. I am aiming stright at the ground and I hear the Cobra fire but nothing happens. Incredibly irritating.
Hmm. Haven't heard of this grab glitch, sounds scary. I'll add it to the list. If others can confirm it's happened to them as well, and how, that would help. Also what systems it has occured on. Thanks for the detailed information though!

I play Drellguard 90% of the time. Most of that time has been spent using builds that always take rank 4 area charge. But I'm currently playing a build with damage instead. I've been running into a problem where I pull enemies and charge them immediately but about 30% of the time no detonation.
Please don't confuse this with the detonation set up being overwritten with tech powers or other biotics. This happens even when I solo bronze. Pull, charge, no boom. The target goes flying from the force of the charge but the biotic detonation doesn't trigger and the target doesn't die.
I had this issue in the demo with my heavy charge builds. But in retail all my builds used area charge so I forgot about it. But now I find it's still there. With Area charge I detonate biotic 100% of the time unless the pull is over-written by tech powers or detonated by someone else. Without area charge 30% of my charges are biotic duds.

I've seen that, too. Phantoms are a weird lot in the glitch department. They get stuck in walls way up high with only their ass hanging out. Unable to fire at you but they make a nice grunting sounding nonetheless. Melded into the floors and still shoot the bejezuss out of you. At least the stabby doesn't happen then.

things such as bodies of primes and atlases not disappearing after they explode.
cloaking animations not processing for players or npcs.
inability to see tech bursts, biotic explosions, etc.
guns disappearing.
etc.
this seems to happen more frequently when I host, it happens about 50% of my hosted matches.

Survival Packs: Possibly due to lag, it is possible to activate a survival pack but then immediately die. You will hear the sound and see your shield/health come back but then end up on the ground waiting for revival. You lose the pack. This is especially tragic when combine with the other Survival Pack bug, where you can use up multiple packs at once due to the lack of cool-down. PC
Missing Power Animations: Previously mentioned with biotic powers, this also happens with tech powers like cryo blast and incinerate. There is no animation when the power is cast, although it does effect the target. PC

Stasis bug:
I can confirm this on the PC, too. And it can happen to any enemy, I believe.
Apparently, everyone but the host will not see the phantom and only see their bugged body stuck on the ground wherever they were when in stasis. And, yes, the "real" phantom is like a ghost leaving her body and will be completely invisible, often running around right next to you, with no way for players (besides the host, if he can indeed see her) to target her but she will still deal damage to you.
The only "solution," which isn't all that clear if it works 100% of the time, is to re-stasis the bugged body which can reset/realign it to where the phantom actually is. Although, I am unsure whether or not you have to have a line of sight (ability to target her if she were not invisible) with where the phantom's "ghost" is in order for the stasis on her bugged body to affect her.
Teammate melee sound effects volume:
Melee sounds from teammates are too loud. They can be on the other side of the map and get a grab kill or hit an enemy with a melee and you will hear the sound effects at the same volume when you hit enemies with a melee.
Biotic projectiles invisible:
I can also confirm that biotic projectiles often stop being visible once thrown, making it hard to tell whether or not your warp hit an enemy. This seems to happen later in the game (from wave 5 onwards, for example). I have yet to see it happen at the start of a game.
This is all on the PC.

There is a glitch the stops people from rolling forward. Not sure how it occurs, but it can be fixed by going into and out of cover.

-Player will still remain online even though he dropped from the game (and is offline). Happened to one of my game where someone had disconnected and many bugs happened such as invisibility. Our shield would recharge as soon as it took damage. But we were still susceptible to oneshot kill animation from phantom. However the disconnected player was 100% invicible and would respawn as soon as he died.
-Warping in respawn area. During extraction point, I got killed and immediately respawn at the starting area.
- Infinity geth. On firewhite base, one geth got invincible. The host quit the game so we could restart the wave. Geth Kept coming without an end.
