I had accepted Wynne into my party, and went to the bottom of the tower and tried to open the locked door and a "Shah Wyrd" appeared.  I allowed him to slay everyone in the room and when the last mage died it updated my codex and quest journal as if I had slain Wynne, even though she was alive and in my party.
