I think this is the ducklips phenomena that a few of us are having. Is yours an ME1 or ME2 originated femshep, and does she have lip shape 1 on the slider?
I created her in ME1 but did go back to ME2 to recreate her, just to see if it would make a difference. She has lip shape no 8. However, I just realized that after the patch, she imports into ME3 with lip shape 6 (weird?). So I redid her with the correct lip shape, used Lazarus with the recommended .exe and r17 map, and she did turn out a lot better - as in, looking more like a person. I especially like how the eyes turned out. But for some reason, with LP the lips look like they're located lower on the face now, or maybe the nose is shorter - in any case, the nose-lip ratio seems off, which is something I'd like to fix.
