Yeah, it will happen a lot with those eyelashes. In fact, i had clipping issues with them even in ME12, but back then it was pretty rare, and thus didn't bothered me that much.
The way i dealt with the problem was that i have first unmapped upper eyelashes in Lazarus prior to applying changes. Then i opened up edited save game with gibbed and manually adjusted positions of vertices that make up the eyelashes.
I ended up increasing Z-value of top and middle vertices by 0.1 and 0.05 respectively, which resulted in longer eyelashes that do not clip, or at least don't clip quite that often.
If you want to try it, these vertices make up the top of upper eyelashes:
0, 2, 3, 7, 10, 11, 21, 22, 25, 28, 30, 31, 34, 45, 46, 49, 51, 54, 55, 57
And these make up the middle:
1, 4, 8, 9, 20, 23, 24, 26, 27, 29, 32, 33, 43, 44, 47, 48, 50, 52, 53, 56
Or you can simply unmap eyelashes in Lazarus and stick to default ones.
@ Teeth. I don't know the lod values for the teeth, but maybe you can try seting the "mouthbase" a little down. (The Z value 0,1-0,3 further to negative.)
Edit: Ok, after the Pictures... you have a bigger problem with the eyes as just clipping. I would unmap eyes in Lazarus and set HED_EyeShadow_Tint_Scalar to 1.25 or 1.50. Didn't bring back the eyelashes, but enhance the Color.
Then try the solution from the quoted post.

I tried, I think, 4 different maps. It's not so much ducklips as it is "pointy cheek" and a just sort of overall "doesn't look right".

For some reason, when I tried one without lip mapping, her lips got super odd shaped. Also trying to solve the pointy cheeks, and to an extent I think the hair color us off.

Hmm. I'm not sure I see the problem with the cheeks. Lazarus doesn't fix the hair color, unfortunately. You'll have to manually correct it.

It really makes Chris' comments about different lighting etc ring hollow when outstanding members of the fanbase can fix problems that bioware aparently cannot.

Yeah, I know I'll have to fix the hair color manually. Just hard to get right. What I'm talking about, about the cheeks, was mentioned earlier in the thread by other people. When I use the r17/18 map(s), it nets that image from my screenshot, but depending on the conversation angle, part of her cheeks are puffed out, and kind of pointy.
When I tried the map you had up recently, the one without lip mapping, it did something to her lip appearance that I can only describe as "simian".
So that's pretty much where I'm stuck right now.

Not yet, but I'll try in a bit, see if it fixes monkeyface Shep. With my renegade, it's that I gave her such a unique look, (IMO) I think. Makes it very easy to tell if it's even slightly off.
Just been trying to get that a-ha moment, the one where you look at your Shep, and for the first time, it feels right.

My point was simply that fans made better translations than Bioware.
