 The simple "Got Out of Hand" theory
BioWare gave to many choices in ME1/ME2 and some in ME3, choices over choices over choices. In the end all those choices had to come together, but so many of them made the choice convergence extremely hard to implement.

Aiming for multiple endings, reflecting all the previous choice branching, apparently prove to much for BioWare. In the end most of choices led to dead ends or became inconsistant.

ME3 ending was a sloppy job, the game itself is not that good also, this of course by BioWare standards. Dragon Age 2 was an uninspired game but ME3 comes as a "Oh well we have to conclude the story". Lack of care/attention and amateurish details are all over, starting in the charater import or on the femshep running animation...I cannot believe they are happy with the game, because this type of things happening are...against BioWare creative integrity.

Mass Effect grew bigger than BioWare expected initialy, and it got out of hand.

And I'm sorry BioWare, but since we know that you can deliver, if you didn't it was because you didn't want to (No one believes this one) or because you were not comited enough to the herculean task you had on your hands. I'd like to think that you have a new project in your hands and are so thrilled about it that you unintentionally relegated ME3 to a secundary task.

 In your comendable purpose of making a game in SYNTHESIS with your fans, you made a mistake, well **** happens to the best. Just don't relegate this 100,000 core fans you have to a secondary plan in favour of the 2 or 3 or 4 million that although buying the game as we did are already playing something else.

 And we that are not happy with the game should give them some peace. Bioware guys may have acted like "****s" on this one but they are our "****s"...
