There's the memory band of course, that you get from the Collector's Edition.
The only other one I've found is a belt for sale at the magic shop in Denerim that increases XP from codex entries. (I don't think all codex entries give XP though, just books).
Are there any other such items?

Right... not all codex entries give XP. Just books in the game world that you click on, as far as I know.
