when you see a teammate that doesn't know to keep distance with Banshees. Once, I witnessed all three of my teammates got killed by one single banshee, and surprise, I can't revive them. I mean, dudes, keep away from the banshee!!!
You know you are going to lose a silver match when.....

When your supporting Asari adept sporting a Widow runs off to some unknown location on wave 9 to fight guardians while the phantoms pour in from the usual spots. Ends in tears. Always.

Every Banshee and Brute completely ignores the fact you're cloaked, regardless of whether you just come round a corner but were already cloaked, just cloaked, or are considerably further away than they can reach...

A few I've encountered...
* When the host ninja-switches to Silver despite dying in Wave 3 on Bronze.
* When you start the match with three Snipers with one backup weapon between them and not one of them is an Infiltrator. Especially on Reactor.
* When your team decides to focus down the Large Bullet Sponge (Brute, Atlas, Geth Prime) across the map and ignore all the little things busy killing you.
* When you realize your Infiltrator has no intentions of moving from his sniper perch, ever.
* Whenever I'm in #1 or #2 after 5+ waves. I'm not silver/gold material.

When someone asks: "How do you use your powers?" on wave 1 because their brother/sister is letting them play on their profile to try out the game.
When the Vanguard uses the Biotic Charge at a group of enemies consisting of a Brute, Ravager, and Banshee on the complete other side of the map.
When you searched for a Bronze level challenge and have a level one character because you just promoted them five seconds ago and you're trying to level him up slowly and only have one offensive power and the host switches to a Silver challenge while you're messing with weapons and equipment and weren't paying attention.
