I know that she wasn't part of the Normandy crew, but I remember finding it weird that a "Talitha" was on the wall, and that the Mindoir-origin Shepard had a small event with a Talitha. And Talitha didn't show up in the third game. I remember wondering if that was some acknowledgment that she died, or if someone on your crew just happened to have the same name.

ON my femshep renegade, Kelly Chambers is on there because she was liquified by the Collectors. She never survived ME2!!! If you have Miranda, Mordin, and Samara as the survivors of ME2 you can get them all killed in ME3. I know...I got them all killed (actually shot Mordin in the back...so I guess that's more like killing him point blank then getting him killed)...

Buy all the ME2 DLC, make sure everyone dies in the suicide mission possible (don't help the crew) by not doing loyalty missions and not getting upgrades.
Kill VS on the Citadel, kill Wrex by sabotaging genophage. Should do it?
