I have finished the Deep Roads stuff 4 times now, each time clearing LITERALLY every single enemy and looting every single treasure I found, yet I haven't seen an "Ancient Darkspawn," nor have I been able to complete Topsider's Honor.
I have found both the pommel and the hilt all 4 times.
What gives?
Why can I never complete Topsider's Honor??

You also have to find the gravesite off one of the caverns in Ortan Thaig before you meet Ruck and click on the boulder that marks the grave.

First off - this quest is kinda stupid - you'll find out why.
Okay - here goes.
First of all - You'll have to find the gravesite which is in the orthan thaig in a slightly "hidden" sidecavern in the very south eastern part of the map.
Secondly - You'll have to find the POMMEL in Ruck's Cave in the very north eastern corner of the Orthan Thaig Map
Third - You find the HILT on a Genlock in Caridins Cross (Not exactly sure where - but if you just kill everything you'll be fine)
Fourth - Once you did all those three an Ancient Darkspawn appears in a corridor in the Dead Trenches. He will drop the BLADE.
Once you've gathered the BLADE (From the Ancient Darkspawn), the HILT (From the Genlock in Caridins Cross) and the POMMEL (From a Vase in Rucks Cave), you have to return to Orthan Thaig to the Graveside to "turn in the three parts" and get the finished Sword.
The stupid thing is that you actually have to return to the Grave... a lot of walking.
Hope I could help.

I have found both the pommel and the hilt all 4 times.
If I'm not mistaken, there are three parts to the Topsider's honor. Pommel, hilt & blade.
One of the pieces can be found on an emissary in Carridin's cross, the second piece can be found in a vase next to Ruck, the third piece is in possession of an elite Darkspawn in the Dead Trenches. Once all three pieces are acquired, you have to bring them to the Topsider's grave, whereupon you receive the blade.
I *think* that's about it. Hope that helps!
Edit: oops, csmayer beat me to it!

He states that the ancient darkspawn does not appear. Which is where he should get the blade. It happened to me once, but I just went on playing, so I'm no help.

If you already have the pommel and hilt - you have to go find the gravesite in Orthan Thaig.
It is possible to find both hilt and pommel before finding the grave but you need to find all three in order to have the ancient darkspawn appear.

Yeah I was baffled about this to until I carefully read the quest again and realized I had to go BACK to Ortan Thaig. It probably is the only quest you actually have to backtrack in a dungeon.

But it is a lovely weapon. Orzanmar was my first ally pickup and I ended up finishing the game dual weilding Topsiders honor and Starfang.

I found the pommel and hilt, then stumbled on the grave, before finding the blade. No problems. As for returning to the grave after collecting the pieces? What, you carry a portable magical forge around with you to repair old relics you find deep behind enemy lines?
The stupid part I found, was that I still had the components after fixing the sword. Now THAT, was stupid.

so you're saying that a gravesite is a magical forge to repair old relics you find deep behind enemy lines?
But you're right of course. It would be better if one had to bring the pieces to a smith to "repair" the sword.
The fact that one keeps the three pieces in the QuestItemInventory is very stupid indeed
Still - the sword is totally worth it! agreed @ Elizire

There's no need for a forge, since the blade has already been made. All that's needed is to assemble the pieces.

The back tracking are bad, really bad. The story of the sword is nice, the sword itself...

as in, i need to load from a previous save to get this? yea, no thanks.

It's just that the ancient darkspawn won't show until after you find the grave. If you have the other parts you can just go to the grave and then head to the ancient darkspawn. No need to reload.

It's a good 1H sword imo (many sockets, good damage, etc.) - always gave it to Alistair. Frankly, I don't think the backtracking part is too terrible. If you're smart about it and use the TAB key to make sure to loot everything in existence down there, really, all you do is make the one short trip back to the gravesite. It pays to be obsessive about filling in your map though I guess.
