Two times in a row I had the same glitch:
Challenge is silver. Mission goes as usual, then at some time, I get the note that I can use an ammo pack (key 8) to refill ammo. I use it.
Suddenly, when I run out of ammo again, I'm not able to grab new one from the refill boxes. Also can apparently not be killed, have infinite bombs, can fire that frost beam thing the infiltrator has without recharging. And suddenly am unable to climb ladders. 
I assume it has to do with the ammo pack, but not sure.

Okay, thanks for the hint! Had assumed it was something recent.
