I did that so many times my first run through.  I swear, I only ever found heavy weapons after the fight I was supposed to used them for.  Image IPB Palaven? Killed all three brutes before finding the Blackstar.  Rannoch? Only saw the one Geth Spitfire before I took down all three Primes (turns out there's a whole bunch of them throughout the room). Thessia? Killed the Bannshee, Ravager, et al before seeing the Hydra sitting right there.  I barely used any heavy weapons my first playthrough.

There are several Spitfires!? :o

Yep, there's at least two more if I remember right.  There's the one when you walk into the room, and then there is one on both the left and right side.  They're kind of hard to find when you've got three Primes shooting at you.  Like I said, I only found them after I beat the Primes and was exploring the room.

On Normal difficulty the game is a cakewalk.
I'm not that far in my insanity run, that I've seen all enamies, but those two brutes you get on Palaven have been a menace.
Also on Grisson Academy the whole Cerberus squads you face in the great hall were quite a pain in the butt, too.

It's more challenging than in ME2, because of the many tactical options, the enemies exploit.
You're forced out of cover regularily due to grenades. Smokescreens hinders you when trying to get targets. Well done on this one, Bioware.

I hate the screaming Banshees, with the waves of husks (in your face) a close second.

Freaking Banshees impaling me with their wierd ass claws when I charge them. Doing a backwards roll before you go to town with the shotgun usually works, but sometimes there's some crap in the way of your roll.

Maybe try positioning your squadmates so they draw its attention.  My favorite squad is Ashley + Garrus, and they always seem to draw the Banshee's attention away from me, even if I'm shooting it in the back with a shotgun.  Fortunately, in ME3, your squad is much better at actually staying alive.  I've watched Ashley and Garrus bring down Banshees and Brutes all by themselves.  Compare that with ME2, where squadmates who weren't using the Incisor could hardly take care of one regular enemy on insanity.
